What You’ll Do:

  • Provide initial psychosocial evaluations and ongoing      counseling to patients and families.
  • Offer individual, family, and group support, focusing      on emotional well-being and bereavement needs.
  • Collaborate with our interdisciplinary team (IDG) to      create and revise care plans.
  • Serve as an advocate and resource, connecting families      with community services and support.
  • Educate staff, volunteers, and the community about      hospice care and psychosocial support.

What We’re Looking For:

  • Education: Master’s degree in      Social Work (MSW) from an accredited institution.
  • Licensure: LMSW in New York, or      MSW with eligibility to sit for licensure within 6 months of hire.
  • Experience: At least one year in      individual, family, or group counseling (healthcare or hospice experience      preferred).

Salary Information

$31 - $37 Hourly Wage

Requirements

EDUCATION, TRAINING AND EXPERIENCE: Masters’ degree in Social Work from an accredited educational institution. License preferred but will consider someone preparing to sit for licensure. Incumbent will have one year experience in individual, family and group counseling, preferably in a healthcare or hospice setting.


CERTIFICATION/LICENSURE: Possess appropriate certificate / license to practice as a Licensed healthcare professional in the State of New York. Certification in Hospice & Palliative Care preferred if offered for discipline.
