JOB SUMMARY

To manage the Permanent Supportive Housing (PSH) Sober Living program and lead the supportive services team.


ESSENTIAL FUNCTIONS

1. Value cultural competence and diversity

a. Collaborate with staff, members, and the community to gain the perspectives of others having diverse opinions, abilities, values, beliefs, perspective, and ethnic or cultural backgrounds.

b. Take deliberate steps to increase their own cultural competency by attending trainings, events, discussions, workshops, etc.


2. Facilitates a smooth move-in process for new residents.

a. Receives referrals and conducts interviews for potential residents. Provides appropriate choices to applicants based on their recovery status and shows available units. Facilitates lease signing and all related intake paperwork.

b. Educates residents on housing procedures including paying rent and reporting maintenance problems.

c. Maintains a waiting list and ensures vacancies are filled within the timeframes required by funders.

d. Develops and maintains network of referral sources and community partnerships. Represents the program at community meetings and with external stakeholders.


3. Ensures services are delivered by the PSH Sober Living team with a recovery-oriented, trauma-informed approach.

a. Provides day-to-day supervision of PSH Sober Living Coordinators to achieve program goals of sobriety, housing stability, community integration, and increased resources and support.

b. Provides ongoing training to PSH Sober Living Coordinators on the principles of Housing First, Recovery Principles, Psychiatric Rehabilitation, Motivational Interviewing, and Trauma-informed Care.

c. Ensures that development and implementation of Individualized Service and Relapse Prevention Plans are consistent with organizational values, program goals, and evidence-based practices.

d. Guides staff in providing case management utilizing trauma-informed care, harm reduction, assertive engagement, and a strengths-based approach, in alignment with PSH Sober Living best practices.

e. Guides staff in facilitating groups to provide peer accountability, build recovery skills, fostering a supportive community, and maintaining the living environment.

f. Inspires staff to empower the residents and encourage autonomy and a sense of self-determination.

g. Oversees coordination of services with internal and external providers

h. Supports staff in handling difficult or complex cases.


4. Ensures accreditation and funding source requirements are met.

a. Ensures timely and accurate documentation is completed by the PSH Coordinators (Service Notes, Individualized Service Plans, Relapse Prevention Plans, DLA-20s, etc.)

b. Ensures residents are seen at the necessary frequency based on level of need and funder requirements.

5. Administrative Responsibilities:

a. Coordinates with Property Manager on all property management functions (e.g., leasing, orientation, inspections, maintenance requests, evictions, etc.)

b. Ensures each resident has an electronic client file containing referral information, up-to-date leases, progress notes, financial verification if required, and other pertinent documentation.

c. Assists Director in maintaining and reporting monthly housing data and outcomes.

d. Assists Director in developing and revising policies and procedures.

e. Complies with all funding sources.


6. Supervisory Responsibilities:

a. Orients new staff to program mission, goals, policies, and procedures.

b. Oversees job responsibilities of the PSH Coordinator(s) and the Recovery House Apartment Manager.

c. Conducts and documents weekly supervision, quarterly work plans, monthly audits on service notes, service/relapse prevention plans and performance reviews of supervised staff.

d. Provides clinical oversight to ensure program adheres evidence-based interventions, follows the appropriate service philosophy, NARR standards, etc.


7. Public/Staff Relations:

a. Develops positive working relationships with landlords, neighbors, and community agencies.

b. Works with MDHA and other agencies, committees and organizations as assigned on housing issues.

c. Utilizes voicemail and email.

d. Communicates regularly with other staff to meet the needs of members.

Works as a team player with other staff.


8. Other duties as a Member of Park Center Permanent Supportive Housing department:

a. Leads staff and supervision meetings.

b. Assists with housing events.

c. Participates in on-call rotation.

d. Maintains good attendance and informs supervisor in advance of annual leave or other time off.

e. Follows all Personnel Policies.

f. Completes required Relias Learning yearly.

g. Other duties as assigned.


QUALIFICATIONS


· Ability to work independently and with a team.

· Education and Experience Requirements

o Must have a high school diploma or GED and at least 5 years experience working in human services field or,

o Must have a bachelor’s degree in a human services field and at least 3 years of experience in a human services field

· At least 1 year of experience in a supervisory role preferred.

· Must be a CPRP or be willing to be certified.

· Must be a Licensed Alcohol and Drug Abuse Counselor or a Licensed Master Social Worker

· Must have a valid Tennessee Driver’s License.


WORKING CONDITIONS

· Hybrid: field work in Nashville, TN, shared office space, remote office capability
