Requirements:
Active RN license (Connecticut)
Current CPR certification
Driver’s license & reliable transportation
Strong clinical, communication, organizational & time management skills
Tech-savvy and comfortable with electronic health records
Home health experience preferred
Familiarity with Medicaid, Medicare, Sandata EVV system a plus
Bilingual (Spanish) candidates encouraged to apply – salary incentive available
Must be able to perform physical tasks (lifting, transferring, prolonged standing, etc.)
Key Responsibilities:
Conduct comprehensive patient assessments and develop individualized care plans
Reevaluate patient needs and update care plans accordingly
Provide hands-on nursing care, administer medications, and educate patients/families
Coordinate care and act as Case Manager for your assigned caseload
Assist with discharge planning and health instruction
