Hours: Monday - Friday, 8:30am - 5:00pm

Location: Onsite

PRIMARY PURPOSE

The Clinical Support Assistant is a Licensed Practical Nurse. The Clinical Support Assistant coordinates patient care services through collaboration with LCH&D health care providers, staff and patients. Though a large portion of care and resources will be provided telephonically, clinical care on the premises may also be required at times when performing the role of Clinical Assistant.


ESSENTIAL FUNCTIONS

Daily responsibilities include:

  • Reviews incoming clinical messages from the EMR and the patient portal and prepares provider tasks for follow up
  • Communicates outgoing clinical messages and test results from providers to patients according to Medical Patient Tracking Protocol Policy # 500.19
  • Schedules patients for follow up when requested by providers via the tasking system to discuss test results 
  • Triage of incoming clinical calls per clinical judgement and guidelines per policy # 500.07 When to refer caller to the nearest Emergency Room
  • Communicates effectively to answer clinical questions within the LPN scope of practice and responds in simple non clinical terminology
  • Provides clear, precise, non-subjective documentation in the patient medical record 
  • Make follow up contact with patients as necessary to ensure compliance with recommendations. 
  • Gathers all information to ensure that all relevant details are considered before forwarding task to provider
  • Develop positive, professional, long term relationships with patients and their care givers. 
  • Perform role as Clinical Assistant as needed.
  • Promote good health throughout the facility in conversation and behavior.
  • Must maintain accurate records. 
  • Comply with HIPAA guidelines as all times
  • Must maintain infection control standards at all times per organization policy. 
  • Must have knowledge of the LCH&D Continuous Quality Improvement Plan and be guided by its goals and objectives in carrying out the role of Clinical Support Assistant, LPN.


EDUCATION AND CERTIFICATION REQUIREMENTS

  • Must have current Ohio LPN license.
  • Must have active BLS Certification.
  • Must meet CEU requirements mandated by law to maintain license.


SKILL AND EXPERIENCE REQUIREMENTS

  • A minimum of three years of experience in a non-profit community healthcare environment is preferred. 
  • Must have proficient computer skills and the ability to become proficient in software programs specific to LCH&D i.e. NextGen, EPM, EHR, etc.
  • Ability to maintain a calm and professional demeanor and communicate enthusiastically with patients. 
  • Ability to be responsive in working with a culturally-diverse patient population.
  • Ability to independently coordinate multiple tasks. 
  • Ability to work cooperatively with others. 
  • Must have valid driver’s license and satisfactory driving record. 
  • Bilingual Spanish speaking is not required, but desired. 


PHYSICAL DEMANDS 

  • Fine and gross motor skills will be frequently employed. 
  • Some lifting of supplies and equipment up to 50 lbs. 
  • 20% walking or standing, 80% sitting at a desk. 
  • Category I re: exposure to blood, bodily fluids, and communicable diseases.
