We’re Hiring: we are seeking a full-time Nurse Practitioner or Physician Assistant experienced in Skilled Nursing Facilities to join our team as a Regional Lead. This role is a combination of clinical practice, regional oversight, and business development, responsible for ensuring clinical excellence and growth. The ideal candidate will be a strong clinician, mentor, and relationship builder in skilled nursing.


Day-to-Day Responsibilities
Primarily responsible for regional oversight of clinical care and facility relationships, you will also round 2-3 days a week at your own designated facility to care for patients who are experiencing difficulty participating in therapy due to pain or functional limitations.

Key responsibilities include:

  • Clinical point of contact for all Providers in your region
  • Clinical training to ensure all Providers display excellent clinical decision making and bedside manner
  • Time shadowing each Provider in the region
  • Regular clinical and facility meetings with facility staff
  • Focused assessments and developing individualized treatment plans in collaboration with facility primary care teams to optimize therapy participation and functional recovery.
  • Providing targeted interventions such as joint injections, Botox treatments, dry needling, and medication management based on patient-specific needs.
  • Supporting physiatrists on-site and operating with a high level of autonomy
  • Coordinating care and referring to other specialties or services for issues beyond the scope of physiatry

This role is best suited for a self-motivated and confident clinician who thrives in independent practice environments and is passionate about improving rehabilitation outcomes.

Qualifications:

  • Current, unrestricted license as a Nurse Practitioner (NP) or Physician Assistant (PA).
  • DEA licensed or willing to obtain DEA licensure
  • Nursing home and/or physiatry experience
  • Comfort and interest in performing peripheral joint injections (knees, shoulders, hips).
  • A genuine passion for providing professional, personalized care in an autonomous environment
  • Strong communication skills, with the ability to collaborate with multidisciplinary teams
  • Familiarity with EHR charting
  • Experience in a clinical leadership or supervisory role
  • Willingness to travel regularly within the assigned region
