Key Responsibilities
Conduct comprehensive biopsychosocial assessments and develop individualized service plans.
Provide outreach, service coordination, advocacy, and supportive case management in community settings including homes, shelters, and agency offices.
Link individuals to medical, behavioral health, educational, vocational, peer, and self-help resources.
Coordinate care among multiple service providers to promote recovery and optimal self-sufficiency.
Utilize trauma-informed and harm-reduction approaches in service delivery.
Provide crisis intervention for assigned and non-assigned clients when needed.
Facilitate discharge planning, aftercare coordination, and follow-up services.
Maintain accurate, timely, and legible documentation in narrative and data formats consistent with agency standards.
Develop and maintain collaborative relationships with colleagues and community partners.
Participate in supervision and ongoing professional development, including cultural competency growth.
Protect client privacy and confidentiality in accordance with agency policy and applicable regulations.
Perform additional related duties as assigned.
Qualifications
Bachelor’s degree in social work, psychology, or related human services field from an accredited college or university.
Experience in case management, behavioral health, or related services preferred (internship and volunteer experience considered).
Valid Pennsylvania driver’s license, reliable transportation, and appropriate insurance required.
Ability to obtain and maintain required clearances (FBI/Fingerprinting, PA Criminal, PA Child Abuse) and pass a drug screen.
Computer proficiency in Microsoft Office Suite; experience with electronic health records preferred.
