Responsibilities:

  • Understands and Manifest the Philosophy and Mission of Ascension DePaul Services.
  • Appointments are scheduled for patients that come through secure messages.
  • Completion of Medical Records requests.
  • Performs initial Client assessment (history, vital signs, current medication immunization status, drugs allergies, etc.)
  • Administration of vaccines for Children.
  • Performs special tests, as needed, such as EKG and other monitor laboratory procedures. 
  • Provides nursing care as qualified. Refers questions to CMO or in CMO’s absence, to Medical provider.
  • Assists in providing Health education for Individuals and groups both in the Clinic and home setting.
  • Maintains Clinic area in functional order.
  • Assists in obtaining lab studies from outside laboratories and follow-up client information from outside agencies, physicians and hospitals.
  • Provides for networking with other agencies and refers as necessary.
  • Assists with Clinic Record keeping.
  • Maintains and updates professional development in related areas to primary ambulatory care. 
  • Applies compresses, cold packs, hot packs. 
  • Observes patients and reports adverse reactions to medication or treatment to medical personnel in charge.
  • Collects samples, such as urine, blood, and sputum, from patients for testing and performs routine laboratory tests on samples. 
  • Maintaining of supplies for the clinic and the pharmacy.
  • Sterilizes equipment and supplies
  • Inventories and requisitions supplies
  • VFC Primary Coordinator and/or Secondary
  • Ordering Henry Schein and Pharmacy
  • Other duties as assigned                                                                  

 

                                                                            Minimum Qualifications 

Education:

To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.  The requirements listed below are representative of the knowledge, skill and/or ability required.  Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions

  • Graduate of an Accredited School of Nursing
  • A minimum 3-5 year previous experience as a LVN
  • Excellent communication skills
  • Bilingual Preferred. (English/Spanish)

  • Within scope of job, requires critical thinking skills, decisive judgment and the ability to work with minimal supervision.  Must be able to work in a stressful environment.
