Position is based at Harbor Community Health Center location in Plymouth. Position requires up to 75% travel to community health centers in MA. Responsibilities: Provide drug therapy and other pharmaceutical care services in collaboration with multi-disciplinary teamCoordinate oral and written consultation with other health care professionals regarding drug therapy selection and managementDevelop patient-specific drug therapy management plans and therapeutic endpointsPerform medication therapy management pharmacotherapy consultations and assessmentsCoordinate care of patients between community health centers and in-house and retail pharmaciesRequires up to 75% travel to community health centers in MA Requirements: Doctor of Pharmacy (PharmD) from ACPE-accredited institutionLicense to practice pharmacy in MABLS certificationPharmacy Residency All qualified applicants will receive consideration for employment without regard to race, color, religion, sex, sexual orientation, gender identity, national origin, disability or veteran status.
