About the Role:

Ponos is seeking a Nurse Practitioner for our Chicago, location and will serve as a vital healthcare provider delivering comprehensive medical care to patients within their home setting.  This role focuses on diagnosing, managing, and treating acute and chronic conditions with an emphasis on family medicine and pain management. The Nurse Practitioner will conduct thorough clinical assessments to develop individualized care plans that prioritize patient safety and promote optimal health outcomes. Collaboration with interdisciplinary teams and effective communication with patients and their families are essential to ensure holistic and compassionate care. Ultimately, this position aims to enhance patient well-being through evidence-based practices and continuous evaluation of treatment efficacy.

Minimum Qualifications:

  • Current and valid Nurse Practitioner license with NPI number listed in Chicago, Illinois and approved within Impact. 
  • Master’s or Doctoral degree in Nursing from an accredited program.
  • Certification in Family Medicine or relevant specialty.
  • Demonstrated experience in clinical assessments and patient management.
  • Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) certifications.

Preferred Qualifications:

  • Board Certified a plus not required 
  • Additional training or certification in Geriatrics. 
  • Familiarity with trauma nursing and emergency care protocols.
  • Experience working in multidisciplinary healthcare teams.
  • Knowledge of electronic health record (EHR) systems and healthcare informatics

Responsibilities:

  • Perform detailed clinical assessments and physical examinations to diagnose patient conditions accurately.
  • Develop and implement individualized treatment plans, including prescribing medications and therapies.
  • Manage acute and chronic pain conditions using evidence-based pain management strategies.
  • Provide primary care services across the lifespan, addressing the unique needs of pediatric and geriatric populations.
  • Ensure patient safety by adhering to clinical protocols and monitoring treatment outcomes.
  • Collaborate with physicians, nurses, and other healthcare professionals to coordinate comprehensive care.
  • Educate patients and families on health maintenance, disease prevention, and treatment plans.
  • Document patient encounters thoroughly and maintain accurate medical records in compliance with regulatory standards.
  • Respond promptly to trauma cases and provide appropriate nursing interventions as needed.

Skills:

The Nurse Practitioner will provide the follow within the patients home and utilize clinical assessment skills daily to evaluate patient health status and identify medical issues accurately. Expertise in family medicine and pain management will guide the development of effective treatment plans tailored to individual patient needs. Skills in chronic illness to ensure that care is appropriate and sensitive to the unique challenges of these populations. Strong nursing and trauma care abilities enable the practitioner to respond effectively to emergencies and maintain patient safety. Additionally, communication and collaboration skills are essential for coordinating care with healthcare teams and educating patients, ensuring a holistic approach to health management.

Salary Information

$115000.0 - $130000.0 Annual Salary
