Job Summary: The CNA will perform duties essentially of personal care and related services to persons in their homes. They will also work with individuals and families to assist them in assuming responsibilities toward self-care. The CNA helps the well family members to carry their share of the responsibilities and encourages the disabled member to participate in the life of the family, in so far as able. This position is open to Full-Time, Part-time and PRN availability.
Essential Functions:
- Demonstrates knowledge and adheres to the plan of care.
- Performs duties as assigned by Case Manager including baths, oral hygiene, nail care, foot care, skin care, pressure relief, dressing patient, feeding and transferring patient.
- Competent taking and recording of vital signs in the EMR, notifies appropriate personnel for abnormal parameters.
- Describes the activities of daily living and demonstrates the ability to use techniques to assist the patient to meet these activities.
- Notifies appropriate licensed personnel when patient complains of pain.
- Positions patient in bed with proper body alignment.
- Assists patient with ambulation as necessary.
- Performs all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors.
- Works as part of the interdisciplinary team.
- Prepares clinical and progress notes within the specified time frame of the Agency.
- Apprehends the regulations for supervisory visits every 14 days and maintains compliance.
- Maintains 12 hours of annual education with annual as required by State, Federal, Accrediting and Agency regulation.
Professional and Regulatory Requirements:
- Completes annual education and maintains regulatory requirements.
- Maintains patient confidentiality and HIPAA compliance.
- Reports to visits on time and as scheduled, completes work within designated time.
- Completes in-services and returns in a timely fashion.
- Attends annual review and department in-services, as scheduled.
- Actively participates in performance improvement and continuous quality improvement activities.
- Complies with all organizational policies regarding ethical business practices.
- Demonstrates effective time management and organizational skills; provides patient visits as scheduled; completes patient care within the designated time.
- Represents the organization in a positive and professional manner. Adheres to dress code, appearance is neat and clean.
- Communicates the mission, ethics and goals of Frontier Home Health.
- Home Health Aide certificate obtained by: completion of a certified HHA course certified by State Department of Health Services.
- Receives on-the-job instruction relative to specific patient needs and continuing in-service training, so that patient safety may be assured and the personal care and homemaking needs of the patient are adequately met.
Skills and Competencies Necessary:
- Comprehensive knowledge of general nursing scope of practice and theory.
- Detail orientation and timeliness when submitting notes within the EMR.
- Basic computer skills.
Qualifications:
- High school diploma preferred.
- Minimum of one year experience in the last two years as a Home Health Aide or Nursing Assistant in a hospital; nursing home or home health agency; or completion of Certified Nursing Assistant or Certified Home Health Aide Program within the last two years.
Physical Demands
- Prolonged standing and walking required
- Ability to lift, move and transfer patients weighing over 50 lbs.
- Ability to identify patient needs, make quick decisions, and work under stressful conditions to meet deadlines.
- Meet patient/family psychosocial needs.
- Requires hand-eye coordination and manual dexterity.
- Ability to utilize durable medical equipment in a home setting.
