What You’ll Do:

  • Coordinate and oversee the interdisciplinary MDS assessment and care planning process
  • Ensure timely and accurate completion and transmission of MDS assessments
  • Maintain compliance with federal, state, and Medicare regulations
  • Collaborate with nursing and interdisciplinary teams to support quality outcomes
  • Monitor assessment timelines and address changes in resident condition
  • Support documentation accuracy and reimbursement integrity

What You’ll Need:

  • Active WI LPN or RN license (unencumbered)
  • Minimum 1 years of recent MDS nursing experience
  • Strong clinical judgment and attention to detail
  • Ability to prioritize and manage multiple deadlines
  • Effective communication and teamwork skills
  • Commitment to high-quality resident care
