DESCRIPTION OF POSITION

POPULATION SERVED

The position involves direct and indirect patient care for a population of patients

between the ages of 18 and older. Age specific experience and/or special training

and/or expertise are required to serve this population.


POSITION SUMMARY

Under the general supervision of the Director of Pharmacy, the Pharmacist will be

responsible for having a working knowledge of all areas of pharmaceutical services

provided by Central Valley Specialty Hospital, including drug distribution systems,

central operations, and IV admixture. She/he will interpret physician’s orders for

medications, compound and dispense prescribed drugs. The Pharmacist will maintain

records on all medication orders filled for inpatient use and compute and record

charges. The Pharmacist will evaluate pharmacological, chemical, or physical

incompatibilities in physician’s orders, and communicate the evaluation to physicians.

She/he will participate in distribution of medications to patients as well as educate them

on their medications. She/he will dispense floor stock controlled substances and

maintain inventory records. The Pharmacist will respond to emergency needs of the

department at any time when called upon by the Director or designee. She/he will

advise the Director of malfunctioning equipment, maintenance problems, and any

unusual situations, occurrences, conditions, reactions, or effects including those related

to drugs, drug requests, usage or security. She/he must be able to work cooperatively

with all hospital employees and patients and encourage and maintain good personal

and business relationships with them. In addition to the daily on-site activity, the

Pharmacist will be available as needed for on-call service. He/she may assist in the

teaching of students and house staff where applicable. The Pharmacist must perform

other duties as needed. This position does not provide direct patient care.


 

DUTIES AND RESPONSIBILITES

The Pharmacist is responsible for the daily organization and supervision of medication management activities in their areas, including but not limited to provision of clinical and drug distribution services. The employee performs job duties with sensitivity to the patients’ diverse cultural backgrounds

  • Preparation and dispensing of drugs in the inpatient setting, including sterile compounding
  • Entering medication orders into the computer system and retrieval of patient data
  • Maintenance and review of patient medication profiles
  • Reviewing and verifying physician medication orders and patient medical records
  • Interacting with healthcare professionals and providers to help promote safe medication use
  • Prevent inappropriate or misuse of medications;
  • Provide clinical pharmacy services (pharmacokinetic dosing, therapeutic interchange, IV/PO conversions, TPN per pharmacy, clinical protocols per pharmacy and pharmacist interventions.
  • Supervision of pharmacy technicians, interns, and students (if any)
  • Provide good customer service through compassion and efficiency
  • Inventory and supply management
  • Support for the Director of Pharmacy
  • Ensure accurate record keeping and documentation
  • Basic knowledge of computer systems, Policies and procedures
  • Monthly unit inspections and medication pass rounds
  • All other duties as assigned
Requirements:

This position requires graduation from an accredited School of Pharmacy and CA

State licensure. Must meet requirement of 30 hours of continuing education every two

(2) year. Must exercise sound professional judgment using initiative consistent with a

high level of resident care. Requires the ability to work efficiently and under broad

guidelines, and must frequently exercise his/her own judgment in decision making.

Regularly works with confidential information. Requires endurance to perform tasks over

long periods of work hours. The position requires the ability to perform essential

functions without posing a “direct threat” in the workplace



Salary Information

$60.00 - $70.00 Hourly Wage
