SUMMARY:
The Quality Supervisor collaborates with the Assistant Program Directors and Program Supervisors and supports front line staff to ensure compliance with Office of Mental Health (OMH) and Department of Health (DOH) guidelines and facilitates improvements in quality measures and outcomes. The position will serve both the Bay Shore and Amityville Adult Care Management (ACM) teams and focus on mentoring staff in the engagement of and improved outcomes with high-risk clients.

RESPONSIBILITIES:
- Use the Health Home dashboards to oversee and monitor the timely completion of Gaps in Care, Transitions of Care, Hospital Alerts, and Health and Recovery Plans (HARP) assessments daily.
- Monitor Care Management Agency (CMA) dashboard to ensure that care plans and comprehensive assessments are up to date.
- Collaborate with Assistant Program Directors and Program Supervisors to ensure that charts that have deficits are identified and plans are put in place to improve quality measures in a timely fashion.
- Train new staff as part of onboarding process.
- Train and mentor Care Coordinators in working with high-risk clients to help reduce unnecessary hospitalizations by ensuring that quality measures are addressed timely.
- Provide training and mentoring on evidence-based models, such as motivational interviewing, and support staff on strategies to effectively engage difficult to serve clients.
- Work with Program Directors on Health Home and OMH audits with the goal of continually improving results.
- Ensure consistency in processes between both program sites.
- Run PSYCKES monthly reports to ensure that Health Homes+ clients are served as required.
- Work with Care Coordinators on engagement challenges.
- In coordination with Program Coordinators and staff, conduct field visits as necessary.
- All other duties as assigned.
QUALIFICATIONS:
A bachelor's degree in a health-related field is required; a master’s degree is preferred.
A minimum of 5 years of experience working with youth and/or adults diagnosed with mental and/or physical illness.
At least one year of previous supervisory experience is required.
Knowledge of community resources and experience with or strong familiarity with Case Management is also required.
Proficient computer skills, including Microsoft Office required.
Demonstrated strong clinical, judgement, leadership, and organizational skills are required.
Strong verbal and written communication skills required.
Travel is required, including between the Amityville and Bay Shore locations.
Valid and clean New York Driver’s License is required.
PHYSICAL REQUIREMENTS:
This position often requires sitting and working at a computer for extended periods of time, as well as travelling to different FSL offices, community meetings, home visits, etc.
