POSITION SUMMARY:    

Provides home health nursing care and health teaching to adults in agency service area in accordance with agency policies and procedures.  Oversees and manages the plan of care, physician orders for treatments and services of patients as assigned.  Maintains professional development.  Performs on call and after hours duties as assigned to meet need of the agency. 

 

JOB FUNCTIONS:  (This list may not include all duties assigned.)

 

1.     Utilizes sound clinical judgment and appropriate assessment skills in the provision of nursing services for home care.  Reports abnormal findings or additional needs to the physician in charge of patient’s care in a timely manner.   Makes referrals to other disciplines or services when ordered. 

2.     Maintains awareness of McLean continuum programs and services such as Lifeline, Meals on Wheels, Adult Day Care, etc., and makes referrals as appropriate an desired by patient/family.

3.     Adheres to infection control and exposure prevention practices as outlined by agency policies and procedures.  Maintains appropriate supplies and demonstrates bag technique during home visits.

4.     Develops and documents the plan of care upon admission to service for each patient. 

      Documents assessment and care provided thoroughly and accurately.  Completes and

      submits required documentation in a timely manner and in accordance with agency policy

      and clinical protocols.  Documents revisions to the plan of care as goals are met and/or

      additional interventions are identified and ordered.

5.     Provides nursing care in accordance with plan of care as well as agency policy and procedure and accepted standards of practice.  Assesses learning needs of patients and families and utilizes effective verbal and/or written methods to achieve learning goals.

6.     Implements the physician ordered plan of care using available resources with awareness of payment source and agency practice guidelines.  Works with agency personnel to obtain authorization for all visits, supplies prior to providing to patients.

 7.     Continually evaluates effectiveness of the patient’s plan of care for skilled and home health aide services.  Revises plan of care at least every 60 days and as necessary.  Regularly evaluates the patient’s progress towards goals and initials discharge planning and termination of services when appropriate.  Communicates discharge plans to patient and family as appropriate.

8.     Coordinates care and documents case conferences with other disciplines, multidisciplinary team meeting notes in accordance with agency standards.

9.     Obtains verbal order for continuation of agency services when ongoing skilled need remains.  Completes re-certification assessments and documents modifications to the plan of care in accordance with regulation and agency policy.

10.  Evaluates safety and appropriateness for delegation of tasks/duties when establishing plan of care for home health aide services.  Provides instruction and ongoing supervision to home health aides under established HHA plan of care for patients on caseload or within territory in accordance with state licensure regulations and agency policy.  Delegates initial instruction of HHA to therapist when appropriate, for cases with no nursing services ordered.

11.  Provides feedback on home health aide performance to the HHA Supervisor and the Quality and Reimbursement Supervisor.

12.  Participates in agency Quality Improvement Program.  Identifies areas for improvement within agency processes as well as personal/professional learning needs.  Regularly attends staff meetings, in-services and meets competency and education requirements of position.  Participates in the orientation and training of nursing staff as assigned. 

13.  Demonstrates accountability and responsibility in daily work.  Communicates concerns, questions and needs to appropriate persons(s) in an effective and professional manner. 

      Accepts supervision, guidance from supervisor/director.  Actively works to meet job

      performance criteria and goals.  Demonstrates problem-solving focus during daily work. 

14.  Demonstrates ability to perform job responsibilities in an organized, efficient manner.

      Meets established productivity standards for department.  Communicates changes in

      schedule, needs for assistance or availability to assist other team members in advance

      whenever possible.  Prioritizes duties daily to complete work during established work

      hours.

15.  Performs on call and week-end work as assigned. 

16.  Performs other activities within the scope of competency and license to practice as directed by the Director of Home Care.

 

WORKING CONDITIONS:

Typical Physical Demands: Requires occasional lifting of greater than 80 pounds, often combined with bending and turning. Requires ability to walk up and down stairs.  Requires corrected vision and hearing within normal limits.  Requires ability to travel to and from cases including the operation of a motor vehicle.  Requires use of office equipment such as computers, telephones, fax machines and copy machines.  Requires use of medical equipment in the clinical setting.  Requires eye/hand coordination and

manual dexterity.  Requires frequent prolonged standing, walking, and the ability to perform physical labor for an eight-hour day.

Typical Working Conditions: Work is performed in facilities, private homes, and an office environment.  Contact with staff, clients, families, physicians, police, clergy, ambulance crews, pharmacists, vendors, volunteers, and students.  May be required to work with uncooperative and combative residents.  Exposure to communicable diseases, medicinal preparations, toxic substances and conditions common to the home environment.

 

EMPLOYMENT REQUIREMENTS:

Knowledge, Skills, Abilities:

Knowledge of nursing theory and practice.  Knowledge and competency in the nursing process, quality assurance, infection control, hospice philosophy, restorative, rehabilitative and nursing practice and theory.  Ability to assess, plan, implement, evaluate, and document care and instructions.  Knowledge of community resources.  Ability to analyze data from client outcomes to modify care.  Demonstrates personal integrity and empathy and fosters such in others.

 

Education:

Graduate of an accredited Registered Nursing program.  BSN preferred.

 

Certification/Licensure:

Current Connecticut State License in good standing as a Registered Nurse.

IV Therapy certification desirable

 

Experience:

Home care experience preferred, as well as minimum of one year of acute care experience.
