About this position
The Care Manager is responsible for managing client care and caregiver services to ensure high-quality, compassionate in-home non-medical support. This role oversees a client caseload and is responsible for but not limited to client assessments, care planning, caregiver coordination, scheduling, and ongoing communications to ensure client needs are met safely and effectively.
Requirements:
Client and Caseload Care Management-
· Conduct client assessments and develop individualized care plans.
· Coordinate client intake into services, on-going service delivery and discharge from services.
· Monitor client progress and adjust care plans as needed.
· Communicate regularly with clients, families, healthcare providers and payer sources.
· Maintain accurate documentation and client records.
· Ensure timely communication of any changes in clients’ condition.
Caregiver Coordination and Management:
· Assist in recruiting, interviewing, onboarding and coordination of new caregivers.
· Provide training, supervision and performance feedback to caregivers.
· Maintain accurate caregiver and client schedules.
· Participate in on-call rotation as assigned.
· Participate in sponsored company caregiver and client events.
· Fill in as a caregiver when needed or as assigned to ensure care for the client.
Operational Compliance:
· Knowledge of all the company’s service delivery functions in compliance with federal and state regulations, contractual requirements, accreditation standards, and agency mission, vision and values.
· Document all client and caregiver interactions.
· Follow all company policies, confidentiality standards and reporting requirements.
· Support Operational needs and special projects as assigned.
· Ensure all client and caregiver records are maintained and updated.
· Participation in audits, plan of correction and efforts to increase documentation compliance.
QUALIFICATIONS:
Required-
· Fluent in English and Spanish.
· At least two years of experience working in management or supervisory roles
· Bachelor’s degree in social work, gerontology, public health, psychology, nursing, or related field. Will substitute 5+ years of equivalent experience for education.
Preferred-
· At least two years of experience working with seniors, adults with disabilities and or populations with diverse needs.
· Certified Nursing Assistant Certification
· CPR, First Aid and Basic Life Skills training.

Additional Notes:
· This position has on-call responsibilities and has hours that are outside of the normal working day and requires travel 20-25% of the time. Reliable transportation, a valid driver's license, and valid vehicle insurance are required.
