- Provides daily personal hygiene and care, including oral hygiene (denture care and brushing of teeth).
- Provides grooming of hair and fingernails, including shaving and eye care and eyeglass care.
- Cares for continent residents by toileting at frequent intervals. Cares for incontinent residents by changing briefs every two hours and as needed.
- Straightens room unit including drawers and closets, ensures that resident has adequate clothing and supplies at all times. Makes sure that bathroom, sink, and floor are clear of objects.
- Makes beds in a neat and professional manner changing all soiled linen and removing all soiled linen and clothing in an appropriate container.
- Ensures proper positioning of all residents while in bed or in the wheelchair, making sure that all mattresses, pads and positioning devices are in place.
- Answers all call lights immediately, and uses proper body mechanics and asks for assistance when needed.
- Makes rounds every two hours or as needed on all residents and ensuring that resident is receiving range of motion exercises.
- Maintains resident's privacy at all times, knocking on doors, closing doors and closing curtains while performing personal care.
- Sets up meal tray for residents when it is needed.
- Records amount resident has consumed accurately. Picks up trays.
- Maintains and records calorie counts accurately. Distributes fresh water to residents every shift. Distributes supplements as ordered.
- Feeds or assists in feeding as direct using proper utensils and instructions per Speech Therapy recommendations.
- Maintains isolation and precaution procedures according to protocol. Maintains proper Foley catheter procedure and care.
- Carries out Restorative programs and charts the progress of the residents as directed. Encourages all residents to remain as independent as possible.
- Practices proper hand washing technique and ensures that gloves are used whenever bodily fluids are involved. Removes gloves when leaving room and disposes of properly.
- Uses proper body mechanics at all times, utilizes gait belt or Hoyer lift for all transfers.
- Operates and uses all equipment in a safe and proper manner.
- Answers telephone in a courteous and professional manner identifying self. Answers and speaks to all residents, families and staff in a courteous and professional tone.
- Measures and documents vital signs, weights, intake and outputs in an accurate manner and records appropriately.
- Charts electronically and on sheets (as needed) at appropriate times and at completion of tasks.
- High School Diploma or G.E.D.
- Illinois CNA State Certification
- Minimum 6 month experience in LTC/SNF preferred
