Compensation: Dependent on Experience

Schedule: On-site Monday through Friday. 8-hour day shift. No nights, no weekends, and no major holidays!

About the Role

The Patient Care Navigator is a key part of Envita's patient experience, serving as a trusted first point of contact for individuals exploring our specialized care programs. This role focuses on education, connection, and care navigation - helping patients and their families feel informed, supported, and confident as they consider next steps.

This is a primarily phone-based position, engaging in warm, inbound conversations with patients who have already expressed interest in Envita's services. These are meaningful, relationship-driven conversations - not cold calls - often with individuals navigating complex medical decisions or emotionally challenging circumstances. Your role is to meet each patient with empathy, clarity, and thoughtful guidance.

This is an in-office position located at our Scottsdale, AZ campus, working alongside a collaborative, mission-driven care team.

What You'll Do

  • Connect with new and prospective patients who are actively seeking information and support
  • Educate patients and families on Envita's personalized care approach in a clear, compassionate, and approachable way
  • Listen closely to each patient's unique situation and tailor conversations to where they are in their journey
  • Translate complex medical or technical information into language that is easy to understand
  • Build trust and rapport through empathetic, patient-centered phone conversations
  • Guide patients through next steps and help to coordinate their care experience
  • Collaborate with internal clinical and administrative teams to ensure a seamless patient journey
  • Accurately document conversations to support continuity, quality, and personalized care

Medical or healthcare experience is not required. Comprehensive training is provided. What matters most is your ability to connect, communicate, and care.

Preferred Experience

  • Phone-based roles such as patient intake, care coordination, customer support, client services, or consultative positions
  • Experience supporting individuals during stressful or emotionally complex situations
  • Backgrounds in healthcare, wellness, advocacy, or service-oriented fields
