About this position
RESPONSIBLITIES:
- Provide services including transporting as needed for assigned residents included but not limited to: scheduling and attending appointments to meet the needs of the residents, i.e. education, medical, dental, psychological evaluation, psychiatric and medical hospital admittance, local juvenile authority, family visits, etc.
- Completing an admission assessment or any other evaluation of a resident for placement
- Developing, reviewing, and updating of service plans for a resident in care
- Completing a discharge or transfer summary for a resident
- Approving any restrictions that will be imposed on a resident for more than seven days that have not been reviewed and approved by the treatment director or service planning team, and any monthly re-evaluations of restrictions that continue for more than 30 days
- Approving any restrictions to communication and visitation with the resident’s family that are imposed on a resident, but have not been reviewed and approved by the treatment director or service planning team, including monthly re-evaluations of restrictions that continue for more than 30 days
- Approving any restrictions to a particular room or building for more than 24 hours that are imposed on a resident, but have not been reviewed and approved by the treatment director or service planning team
- Ensuring the provision of planned but flexible program activities designed to meet the developmental needs of residents
- Ensure on a daily basis the safety and cleanliness of all areas of the program. Adhere to all regulations and standards necessary to meet licensing and accreditation compliance.
- Complete a minimum of 50 hours on in-service training hours annually in child care services, excluding first aid and orientation.
- Administer medication and keep accurate records.
- Maintain resident records
- Responsible for contractual agreement with outside agencies
- Intake of assessments
- Meeting the Campus Census Requirements(48 hour time window to fill upcoming vacancies)
- Psychiatric and Medical Hospitalizations (all steps of the process)
- Meeting deadlines as required per our multiple contracts ( DFPS, OCOK, Family Tapestry, 2INgage, Post Adoption), CARF, Minimum Standards and YFT
- Assist with training as it pertains with/to your area of expertise
- Participation in the Performance Appraisal process including a review of your Job Description and completion of job responsibilities
- Other duties as assigned

The Case Manager reports to the Child Care Administrator and the Clinical Director.
Requirements
Minimum Qualifications
- Bachelor’s Degree with a minimum of 2 years experience in residential treatment
- Knowledge of minimum licensing standards
- Must work a minimum of 50 hours per week
- Must be on call 7 days a week
