POSTION SUMMARY:

The Adoption Case Manager position specializes in and offers expertise in adoptive placement home and child studies under the direction of the Program Director or Assistant Program Director.

WHAT YOU WILL DO:

  • Provide on-going case management services until the adoption process is completed.
  • Offer a variety of adoption-related services for waiting children and their Case Manager.
  • Coordinate child-specific recruitment plans with the Adoption Supervisor and Case Manager.
  • Arrange match meetings with potential adoptive families and other parties involved in the child's care.
  • Complete adoptive home studies and updates for prospective families.
  • Use of a personally owned vehicle may be required.
  • Demonstrate knowledge of community resources.
  • Assess, interpret, and deliver care-related information to prepare a child and family for adoption.
  • Must possess a valid Florida Driver’s License and be insurable by the agency.
  • Must be computer literate.
  • Communicate effectively, both verbally and in writing.
  • Document clearly and concisely in a timely manner.
  • Facilitate group discussions and manage group dynamics.
  • Arrive on time and ready to work.

  • Proficiency in Microsoft Office Suite.
  • Provide case management services for children awaiting adoptive placement, consulting with the Adoption Supervisor.
  • Complete documentation for adoptive placement, including home studies, child studies, subsidy requests, home visits, and case closure.
  • Provide adoptive placement services, ensuring legal compliance for full disclosure and adoption subsidy.
  • Review and update service/case plans monthly to ensure progress toward permanency.
  • Complete monthly progress reports and share with all stakeholders.
  • Update Child Welfare Case Manager on adoption case activity, attend staffings, and testify in court.
  • Perform specialized case management for program compliance with legislative mandates and agency initiatives.
  • Coordinate and participate in sibling separation staffings, match staffings, and Adoption Applicant Review Committees.
  • Assess children and family needs and coordinate adoption preparation, transition, and support services.
  • Collaborate with the Child Welfare Case Manager monthly to meet the needs of children and families.
  • Complete measurable objectives related to adoption goals and meet legal requirements under Chapter 39.
  • Provide families with educational resources on child abuse and neglect to promote family stability.
  • Conduct home visits every 25 days to ensure child safety and family engagement.
  • Participate in staff development programs, team meetings, client care, and utilization reviews.
  • Perform other duties or special projects as assigned by leadership.

WHAT YOU NEED / REQUIREMENTS:

  • Bachelor's degree in human services
  • 2+ year experience in foster care and adoptions or related job preferred.
