Key Responsibilities:

- Provide leadership, direction, and support for team members
- Ensure adherence to clinical and operational standards
- Foster relationships with patients, families, referral sources, and community partners
- Manage caseloads, schedules, and performance metrics as applicable
- Support census growth and quality initiatives

Qualifications:

- Active LMSW license in the state of Texas
- Prior experience in hospice, home health, or healthcare leadership/sales preferred
- Strong communication, organizational, and leadership skills
- Commitment to compassionate, patient-centered care
