What You’ll Do
The Family Nurse Practitioner will carry a panel of Adult and Pediatric patients seeking Primary Care services at Goodwin Community Health in Somersworth, New Hampshire. Goodwin Community Health belongs to a network of Federally Qualified Health Centers (FQHCs) committed to expanding healthcare access to vulnerable populations in Southeastern NH and Southern Maine. The Family Nurse Practitioner will provide clinical services to patients in accordance with approved treatment protocols and recognized standards of medical practice. The Family Nurse Practitioner will engage with clinical and nonclinical services available to patients and Providers within the federally-designed "patient-centered medical home" model of care. This model of care enables GSCH to offer Medication Assisted Recovery (MAR) Care, Prenatal Care, Pediatric Care, Behavioral Healthcare, Mobile Healthcare, Social Work, WIC (Women, Infants and Children), Pharmacy services, and more. Ideal candidates to be considered for the Family Nurse Practitioner opportunity are interested in coordinating care and delivering treatment plans to patients with complex care needs, or at risk of developing complex care needs and seeking integrated care services in an outpatient, community health clinic setting.
Qualifications:
Education: Must hold a Bachelor's Degree in Nursing from an accredited university program
Education: Graduate of an accredited Advance Practitioner Registered Nursing program
Education: Active, unrestricted certification by the American Academy of Nurse Practitioners (AANP) or American Nurses Credentialing Center (ANCC)
License: Must hold current, unrestricted Registered Nurse (RN) license and Advanced Practice Registered Nurse (APRN) license administered by the New Hampshire Board of Nursing
License: Active, unrestricted federal DEA registration and New Hampshire DEA registration, or eligible for federal and NH registration
New Grads encouraged to apply
Active BLS or ACLS certification
