POSITION OVERVIEW
Under supervision of the Program Director, the requirements listed below represent the knowledge, skill, and/or ability required. Reasonable accommodations may be made to enable individuals with disabilities as defined by the ADA to perform the essential functions of the job. The Registered Professional Nurse is responsible for evaluating each consumer’s needs for medical support and assistance on an ongoing basis. Ensure all medical screenings and supporting documentations are addressed and carried out to support the participants in the program. The RN will also conduct workshops and groups to further educate participants and promote independence.
KEY ESSENTIAL FUNCTIONS
- Provides health assessments and medication management services for adults diagnosed with severe and persistent mental illness. Complete pharmacy reviews: on an as needed basis, Administers Psychotropic Medication Treatment.
- Provides individual and group Wellness Self-Management services and other clinical services rooted in evidenced based practice.
- Works as a member of a multidisciplinary treatment team and practices client centered and holistic care.
- Communicates effectively with client, collateral contacts, outside agencies, referring agencies, and/or outside providers as needed for continuity of care.
- Maintains compliant and timely documentation of services, case trackers and related clinical contacts in accordance with OMH regulations and organizational expectations.
- Performs clinical, compliance, and administrative duties as instructed.
- Participates in QA, UR, IRC meetings.
- Carries out all relevant policies and procedures of the agency issued by management in accordance with funding agency guidelines. This does not limit the assignments of duties or exclude the performance of other duties, not mentioned as assigned by the supervisor.
REQUIREMENTS
- Previous relevant experience in a Mental or Behavioral Health environment required.
- RN required.
- Ability to use business acumen to exercise good judgment and decision-making.
- Ability to deal with sensitive issues with diplomacy and discretion.
- Must be able to multitask with strong organizational skills.
- Excellent interpersonal skills and able to communicate both verbally and in written form.
- Commitment to cultural diversity and sensitivity.
- Ability to maintain a professional demeanor under pressure and operate with a keen sense of urgency and commitment to quality.
