About this position
? Summary: Responsible for the timely scheduling, completion, and transmission of MDS assessments and support of the RAI process.
ESSENTIAL FUNCTIONS: ?
Schedules assessments for PPS,OBRA assessments and CAA’s. ?
Completes MDS’s and CAA’s ?
Secondary UR Coordinator at facility. ?
Gathers information for assessments and care plans. ?
Monitors PCC/POCor flow sheets, and physician orders related to MDS completion ?
Assesses or collect datafor residents in conjunction with other team members. ?
Gathers reports as needed: QI/QM’s ?
Support patient/familiy care plan conferences ?
Participates on various committees ?
Inputs or supervises the input of MDS assessments, the transmissions to the state, and obtains receipt of validation. ? Completes/Updates careplans with the RAI process ?
Support nursing services as needed ?
Participates with Medicare meetings, Triple Check in support of Resident Assessment Coordinator ?
Supports Case Mix management and reports ?
Supports LOCD and PAS process as needed ?
Assists with monitoring of documentation for MDS completions and provides training as needed ( ex:ADL’s, restorative) ? Gathers data, completes, or supports insurance pre-authorizations, or re-authorizations in collaboration with case management ?
Collect data on diagnosis codes and enters ?
Other duties as assigned
Requirements
Licensed Nurse in good standing, RN preferred
Acceptable background check Over 21 years of age MDS experience preferred Ability to read, write, comprehend and communicate in English, follow oral and written instructions Must function independently and have flexibility, personal integrity and the ability to work effectively with residents, coworkers, managers, other personnel and support agencies
Able to deal tactfully with people, both in person and over the phone Aptitude and ability to use office machines and computer Able to make independent decisions and possess problem-solving skills Able to exercise discernment and make sound judgments
Knowledge of medical symbols, abbreviations and terminology
Knowledge of nursing and medical practice and procedures, as well as laws, regulations and guidelines which pertain to long term care
