Description:

  • Able to meet job duties and requirements of CNA job description.
  • Observes and reports residents in need of rehab or restorative services.
  • Monitors residents' attendance in programs and records attendance.
  • Carries out all programs assigned to Rehab Department accurately and professionally.
  • Ability to perform Range of Motion exercises to prevent contractures, and ability to instruct the aides in such programs.
  • Ability to apply hand rolls, splints and braces, and the ability to instruct the aides in such applications.
  • Ability to document clearly the residents' progress or decline in the program they are participating in.
  • Weekly Progress Notes are up to date and clearly reflect the residents' status.
  • Assists therapists as needed.
  • Ability to properly use all equipment and instruct floor CNAs on the proper use of all equipment.
  • Encourages residents' participation in rehab programs and continues participation in individual independent goals.
  • Applies preventative devices as warranted by residents' status. Suggests equipment as needed to prevent skin breakdown, contractures or loss of independence.
  • Reports change in residents' condition which may indicate a change of cancellation of a rehab program.
  • Assists in orientation of new employees regarding proper body mechanics and the use of gait belts and equipment.
  • Provides a neat and safe orderly department and environment for the residents.
  • Identifies safety hazards and reports to appropriate personnel immediately.
  • Answers telephone in a courteous manner and identifies self and department.
  • Performs housekeeping tasks as needed to provide a safe and clean environment.
  • Assists the Unit CNAs as needed on the floors. Assists in the feeding of residents at all meals and reports swallowing or dietary problems as warranted.
  • Answers all call lights and assists with residents or staff whenever needed.
  • Monitors supplies and equipment for rehab unit and ensures that there are adequate supplies and working equipment at all times.
  • Ensures that all residents' adaptive equipment is present and in working condition; for example, splints, hand rolls, positioning devices, plate guards, feeding utensils, etc.
  • Participates and presents in-services to staff on a regular basis. Instructs and encourages staff to continue rehab goals when they are necessary.
  • Participate/assist staff in toileting program.
  • Any other duties as assigned by the Rehab Coordinator, Clinical Coordinator or DON.
Requirements:


  • High School Diploma or G.E.D.
  • Illinois CNA State Certification
  • 6-month CNA experience in LTC/SNF or Rehab required


Salary Information

$20.00 - $20.00 Hourly Wage
