What you will do:
The Licensed Clinical Special Worker has the primary responsibility for providing behavioral, mental health and social services under the direction of the Clinical Supervisor of Behavioral, Mental Health and Social Services (CSBMHSS). In accordance with policies, procedures, and protocols established by: standards of clinical social work practice and New Jersey licensing and certification, and other regulatory agencies requirements as appropriate. Responsible for implementing community educational programs, assessing problems and determining appropriate types and methods of treatment, and implementing intervention methodologies; acting as program consultant to staff members with or without professional training; functioning in such areas as counseling, protective services and family services.
General Duties and Responsibilities:
- Assists in the assessment, planning, arranging, and implementation of the behavioral, mental and social health program in the health center.
- Assists in the development of the content of education and counseling programs for individual patients and/or groups of patients and serves as liaison and outreach worker, if necessary, to: community, individuals, and groups as appropriate.
- Educates patients in the health center and the community in the nature of behavioral, mental, and social health related conditions and in the general promotion of the same as it relates to prevention and wellness in the health center.
- Provides short-term therapy for all patient populations (including children, adolescents, adults, seniors, and pregnant patients) served at CAMcare in individual, group, couple, or family therapy as needed by the department to meet the needs of the patients and the community.
- Demonstrates proficiency in the administration of behavioral, mental, social health, and child development protocols sufficient to provide guidance to patients and health center clinicians and staff.
- Demonstrates proficiency in providing crisis intervention, short term intervention, and referral management to approved specialty, secondary, or tertiary providers
- Demonstrates adherence to departmental policies, protocols, and procedures which are in compliance with the most current accepted professional standards and evidence-based behavioral, mental, and social health practices.
- Assists in the coordination and integration of the behavioral, mental, and social health into other programs and services within the health center programs for the welfare of the health center’s patients.
- Conduct surveys and studies related to psychosocial status and patterns of population groups as directed.
- Reviews and evaluates recent psychosocial health research and literature and interprets it to health center clinicians, staff, and patients.
- Performs Peer Reviews in accordance with health center policy and other regulatory bodies’ standards or requirements.
- Prepares and distributes psychosocial educational materials as needed or required under the direction and with the approval of the CSBMHSS.
- Provides in-service presentations and trainings to clinicians and staff as needed or required under the direction and with the approval of the CSBMHSS.
- Contributes to and actively participates in the health center’s QA/PI efforts, the establishment of departmental goals and objectives, and other initiatives presented by the CSBMHSS.
- Assists in the growth and expansion of behavioral, mental, and social services in the health center; and seeks additional opportunities to increase volume, revenue, training, and research.
- Assists in the training of behavioral health staff, social workers, and other program personnel and students as necessary, in: treatment protocols, diagnostic and biopsychosocial assessment, interpretation, treatment planning, behavioral health screenings, crisis intervention stabilization services, and client intervention strategies for patients presenting within the primary care medical home under the direction of the CSBMHSS.
- Works collaboratively with Primary Care Providers, and within the scope of practice, regarding psychotropic medication management. Works collaboratively with mental and behavioral health providers who provide referral and consultative services to the health center’s patients.
- Records in a timely manner per CAMcare’s policies and procedures patient interactions, (both face-to-face, telephonic, or other non-face-to-face encounters as they occur in the patient’s electronic health record) so that the chart accurately and completely reflects the nature of the contact, the condition of the patient, and the care or treatment provided.
- Completes referrals, prior authorizations, data collections, appropriate billing and coding and other records/documentation or paperwork as required.
- Maintains timely records sufficient to meet Corporate, Local, County, State, and Federal clinical documentation requirements, including Medicare, Medicaid, all third-party insurance plans CAMcare participates in, and all other funding sources’ program requirements.
- Actively participates in case conferences with behavioral health, social worker staff, and/or other clinical staff.
- Actively participates in multi-disciplinary team meetings, including primary care providers’ departmental meetings, care coordination team meetings, QA/PI committee meetings, and departmental staff meetings as needed or directed.
- Attends trainings and workshops (which may require travel) as required for professional development and maintenance of licensure, certification, etc.
- Presents self at all times in a professional and ethical manner with demeanor, attitude, dress, communication, and behavior as outlined in CAMcare’s Policies and Procedures and Code of Professional Ethics.
- Keeps all matters related to the organization confidential in compliance with confidentiality policy.
- Accepts and performs other related and/or necessary tasks in order to achieve organizational, programmatic goals and objectives commensurate with the mission and vision of CAMcare as directed.
Qualifications:
- Desire to work with an ethnically and socioeconomically diverse patient population of all ages.
- Bicultural and/or bilingual in English/Spanish, a plus.
- Knowledge of health information related to behavioral health (including substance abuse and addiction), mental health and the social determinants of health (including co-occurring conditions).
- Knowledge of Integrated Behavioral Health models of care, a plus.
- Ability to function effectively, efficiently, and professionally in a high paced, high pressured, collaborative environment.
- Competent in Motivational Interviewing and Cognitive Behavioral Therapy.
- Competent in cultural, linguistic, gender identity, and sexual orientation sensitivity skills.
Education:
- Bachelor’s degree in a closely related field such as Psychology, Child Development, Social Work, or Sociology, a plus.
- Master of Social Work (MSW), required.
- Current Licensure as a Licensed Clinical Social Worker (LCSW), required.
Experience:
- Experience working within a Federally Qualified Health Center delivering care in a Patient –Centered Medical Home model, a plus.
- Experience as a case manager, a plus.
