Nurse Navigator
Ingleside Communities is seeking to hire a Nurse Navigator RN or LPN to join our amazing, dedicated team!
The Admissions and Care Experience Navigator Nurse will focus on providing new patients/residents and families with the necessary coordination of services for an optimal and seamless transition into the facility. This position manages the facility's internal admissions process from intake to discharge and is responsible for coordination of all tours and admissions, to ensure the process is thorough and the patient has a smooth admission, with a focus on their individualized clinical needs and comfort.
This is a full-time position, onsite position.

As our Admissions Nurse, you will:
- Work closely with the Admissions and Care Transitions Manager to support admissions and maintain facility census.
- Coordinate with Nursing, MDS, Social Services, and other departments regarding admissions, bed availability, transfers, and census changes.
- Completes census events in MatrixCare for admissions, discharges, and transfers.
- Ensures that the patient room has been prepared and does final check to assure that it is neatly prepared for receiving the admission.
- Ensure resident rooms are fully prepared prior to admission, including coordination of DME equipment and specialty beds with Central Supply.
- Serve as the patient/resident/family liaison at time of admission and throughout their stay.
- Welcome new residents and guide them through the admission process, ensuring they feel informed, comfortable, and supported.
- Complete admission agreements and consents with resident/POA.
- Complete initial admission assessments and medication reconciliation.
- Conduct follow-up check-ins with residents to ensure expectations are being met and care needs are addressed.
- Conduct insurance revalidations, as needed.
- Work with social services to ensure a safe, stable and successful discharge to the community or next site of care.
- Provide facility tours for prospective residents and families and ensure the community remains tour-ready.
- Follow up on leads, tours, and inquiries in coordination with the Admissions and Care Transitions Manager.
- Participate in weekend on-call rotation coverage.
Requirements:
Active LPN or RN license in the State of Wisconsin
Current CPR Certification
Minimum 2 years of healthcare experience, preferably in long-term care, post-acute, or skilled nursing
Strong interpersonal and communication skills with the ability to build trust with residents, families, and staff
Ability to work collaboratively across multiple departments
Basic computer skills and ability to learn electronic systems such as MatrixCare
