As our Admissions and Discharge Coordinator, youll play a vital role in welcoming individuals served into our programs and ensuring their transition is seamless from admission through discharge.

What Youll Do

Receive, review, and organize all referrals and supporting documents for individuals seeking admission. Coordinate admission and discharge processes with physicians, community referral sources, and facility staff. Maintain clear, ongoing communication with referral partners to ensure timely and accurate information sharing. Track occupancy, discharges, and reconcile bed counts to maintain accurate facility census data. Provide reporting on admission and discharge activity to Administration. Uphold compliance standards across risk management, infection control, and safety protocols.

What Were Looking For

Education:

    High school diploma or equivalent (required)

    Bachelors degree (preferred)

Experience:

    Two (2) years of admissions and discharge experience in a psychiatric or healthcare setting (preferred)
