 We are a newly established hospice organization committed to delivering compassionate, dignified, and personalized end-of-life care to patients and their families in the Milwaukee area.  We are seeking a PRN Registered Nurse (RN) to join our dynamic hospice team. This role is ideal for an experienced nurse who values flexibility, autonomy, and the opportunity to deliver high-quality end-of-life care. You will provide skilled nursing services, coordinate care with the interdisciplinary team, and support patients and their families with compassion and professionalism.

Responsibilities

  • Provide direct patient care in the home or facility setting.
     
  • Assess, monitor, and manage symptoms to promote comfort and quality of life.
     
  • Educate and support patients, families, and caregivers regarding the hospice philosophy and care plan.
     
  • Collaborate with the hospice interdisciplinary team (physicians, social workers, chaplains, aides) to develop and implement individualized care plans.
     
  • Maintain accurate, timely clinical documentation in compliance with state, federal, and organizational requirements.
     
  • Respond to urgent patient and family needs as scheduled or assigned.
     

Qualifications

  • Current Registered Nurse (RN) license in the state of Wisconsin.
     
  • Minimum of 1 year of nursing experience (hospice, palliative care, or home health experience strongly preferred).
     
  • Strong clinical assessment, critical thinking, and communication skills.
     
  • Compassionate, empathetic, and adaptable approach to patient care.
     
  • Valid driver’s license, reliable transportation, and proof of auto insurance.
