Position Summary
We are seeking a skilled and compassionate Advanced Practice Provider to join our clinical team. This role is integral to delivering high-quality, patient-centered care across the continuum of services. The ideal candidate is clinically strong, detail-oriented, and collaborative, with a commitment to clinical excellence and positive patient outcomes.
Responsibilities
- Conducts comprehensive patient assessments, including medical history review, physical examinations, and diagnostic evaluations.
- Develops, implements, and manages treatment plans in collaboration with physicians and the care team.
- Orders, interprets, and reviews diagnostic tests and laboratory results.
- Prescribes medications and therapeutic interventions in accordance with state licensure and clinical guidelines.
- Performs clinical procedures within scope of practice and specialty requirements.
- Provides patient education regarding diagnoses, treatment plans, medications, and preventive care.
- Documents all patient encounters accurately and timely in the electronic health record (EHR).
- Collaborates with nursing, clinical, and administrative staff to ensure continuity and efficiency of care.
- Participates in quality improvement initiatives, protocol development, and ongoing professional development.
Qualifications
- Current and unrestricted Nurse Practitioner (NP) or Physician Assistant (PA) license in the state of practice.
- National certification (AANP, ANCC, or PA-C).
- Minimum of 2 years of clinical experience preferred (new graduates may be considered based on specialty).
- Current BLS certification required; ACLS preferred based on clinical setting.
- Strong clinical assessment, critical thinking, and decision-making skills.
- Excellent communication, organization, and documentation abilities.
- Ability to work independently while collaborating effectively within a team-based care model.
