Description:

We are a newly established hospice organization committed to delivering compassionate, dignified, and personalized end-of-life care to patients and their families in the Milwaukee area. We are seeking a Full Time Registered Nurse (RN) to join our dynamic hospice team. 


The RN provides compassionate, skilled nursing care to patients and families facing life-limiting illness. This role is responsible for assessing patient needs, developing and implementing individualized plans of care, coordinating with the interdisciplinary team, and supporting patients and families through end-of-life care in accordance with hospice philosophy and regulatory requirements.

Requirements:

Clinical Care

  • Conduct comprehensive nursing assessments and ongoing reassessments of hospice patients
  • Develop, implement, and update individualized plans of care in collaboration with the interdisciplinary team
  • Provide skilled nursing care including symptom management, pain control, medication administration, wound care, and patient/family education
  • Monitor patient status and respond appropriately to changes in condition
  • Provide emotional support and education to patients and families regarding disease progression and end-of-life care


Care Coordination

  • Collaborate with physicians, social workers, chaplains, aides, and other members of the interdisciplinary team
  • Participate in IDT meetings, case conferences, and care planning
  • Coordinate care transitions and communicate changes in patient status to appropriate team members
  • Serve as a liaison between patients, families, physicians, and hospice leadership


Documentation & Compliance

  • Complete accurate, timely, and compliant clinical documentation in the electronic medical record
  • Ensure care delivery aligns with hospice regulations, Medicare Conditions of Participation, and agency policies
  • Participate in quality improvement initiatives, audits, and survey readiness activities


Patient & Family Support

  • Educate patients and caregivers on medication management, symptom control, and hospice services
  • Provide guidance and emotional support to families during the end-of-life process
  • Participate in end-of-life care, including pronouncement of death per policy and post-mortem care


Professional Responsibilities

  • Maintain current RN licensure, certifications, and required competencies
  • Participate in ongoing education, in-services, and staff meetings
  • Adhere to infection control, safety standards, and confidentiality requirements
  • Participate in on-call rotation as assigned


Qualifications

  • Current Registered Nurse (RN) license in Wisconsin
  • Graduate of an accredited nursing program
  • Minimum of 1–2 years of nursing experience, preferably in hospice or home health
  • Valid driver’s license, reliable transportation, and ability to travel within service area
  • Current CPR certification
  • Experience with EMR systems and home-based patient care
  • Ability to lift, reposition, and assist patients as required
  • Ability to travel to patient homes, facilities, and meetings
  • Ability to stand, walk, bend, and perform nursing tasks for extended periods
