About this position
We are a newly established hospice organization committed to delivering compassionate, dignified, and personalized end-of-life care to patients and their families in the Milwaukee area. We are seeking a Full Time Registered Nurse (RN) to join our dynamic hospice team.
The RN provides compassionate, skilled nursing care to patients and families facing life-limiting illness. This role is responsible for assessing patient needs, developing and implementing individualized plans of care, coordinating with the interdisciplinary team, and supporting patients and families through end-of-life care in accordance with hospice philosophy and regulatory requirements.
Requirements:Clinical Care
- Conduct comprehensive nursing assessments and ongoing reassessments of hospice patients
- Develop, implement, and update individualized plans of care in collaboration with the interdisciplinary team
- Provide skilled nursing care including symptom management, pain control, medication administration, wound care, and patient/family education
- Monitor patient status and respond appropriately to changes in condition
- Provide emotional support and education to patients and families regarding disease progression and end-of-life care
Care Coordination
- Collaborate with physicians, social workers, chaplains, aides, and other members of the interdisciplinary team
- Participate in IDT meetings, case conferences, and care planning
- Coordinate care transitions and communicate changes in patient status to appropriate team members
- Serve as a liaison between patients, families, physicians, and hospice leadership
Documentation & Compliance
- Complete accurate, timely, and compliant clinical documentation in the electronic medical record
- Ensure care delivery aligns with hospice regulations, Medicare Conditions of Participation, and agency policies
- Participate in quality improvement initiatives, audits, and survey readiness activities
Patient & Family Support
- Educate patients and caregivers on medication management, symptom control, and hospice services
- Provide guidance and emotional support to families during the end-of-life process
- Participate in end-of-life care, including pronouncement of death per policy and post-mortem care
Professional Responsibilities
- Maintain current RN licensure, certifications, and required competencies
- Participate in ongoing education, in-services, and staff meetings
- Adhere to infection control, safety standards, and confidentiality requirements
- Participate in on-call rotation as assigned
Qualifications
- Current Registered Nurse (RN) license in Wisconsin
- Graduate of an accredited nursing program
- Minimum of 1–2 years of nursing experience, preferably in hospice or home health
- Valid driver’s license, reliable transportation, and ability to travel within service area
- Current CPR certification
- Experience with EMR systems and home-based patient care
- Ability to lift, reposition, and assist patients as required
- Ability to travel to patient homes, facilities, and meetings
- Ability to stand, walk, bend, and perform nursing tasks for extended periods
