NCQA offers three levels of PCMH recognition with Level III being the highest level.  As a Level III PCMH, AAPHC is committed to continuously raising the quality of care within our practices, while also lowering our patients health care costs.

Our health care teams are comprised of physicians, physician assistants, nurse practitioners, and nurse midwives. Assisting these providers are clinical staff members, laboratory personnel, and business office professionals. Most medical problems can be handled at our offices, but if a specialist is needed, an appointment will be made for you as a referral from your physician.  

And did you know that AAPHC offers after hours coverage to our patients?  That's right!  A physician is always available by phone after hours for both adults and children. He/she may be reached by calling the number of the office at which the patient is seen. All phones are forwarded to an answering service at the close of each business day, on weekends, and holidays.   Our goal is to always provide access, and even expand access, to your provider so you always can reach AAPHC when you need us most!

About the Role:

The Licensed Practical Nurse (LPN) plays a critical role in delivering high-quality healthcare services within women's and children's health settings. This position involves providing direct patient care, performing clinical procedures such as venipuncture, and assisting in triage to prioritize patient needs effectively. The LPN will educate patients and their families on health management and wound care, ensuring comprehensive support throughout the care continuum. Working collaboratively with registered nurses, physicians, and other healthcare professionals, the LPN contributes to creating a safe and compassionate environment for patients. Ultimately, the role aims to enhance patient outcomes through skilled nursing care, effective communication, and adherence to clinical protocols.

Minimum Qualifications:

• Valid and current Nursing License as a Licensed Practical Nurse (LPN) in the United States.
• Proficiency in reading, writing, and speaking English to communicate effectively with patients and healthcare team.
• Demonstrated knowledge and experience in women's and children's health care.
• Ability to perform venipuncture and provide wound care according to clinical standards.
• Basic understanding of triage procedures and patient care protocols.

Preferred Qualifications:

• Experience working in a healthcare setting focused on women's and children's health.
• Certification in Basic Life Support (BLS) or Advanced Cardiovascular Life Support (ACLS).
• Additional training or certification in patient education or health promotion.
• Familiarity with electronic health record (EHR) systems.
• Strong interpersonal skills and ability to work effectively in multidisciplinary teams.

Responsibilities:

• Conduct patient assessments and assist in triage to determine urgency of care.
• Provide direct nursing care including wound care management and performing venipuncture.
• Educate patients and families on health conditions, treatment plans, and preventive care.
• Maintain accurate and detailed patient records in compliance with healthcare regulations.
• Collaborate with healthcare team members to develop and implement individualized care plans.
• Monitor patient progress and report any changes or concerns to supervising nurses or physicians.
• Ensure adherence to infection control and safety protocols within the clinical environment.

Skills:

The required skills such as knowledge of women's and children's health, triage, wound care, and venipuncture are essential for delivering competent and compassionate patient care on a daily basis. The ability to read, write, and speak English fluently ensures clear communication with patients, families, and healthcare colleagues, which is vital for accurate documentation and education. Nursing license credentials validate the candidate's clinical expertise and adherence to professional standards. Preferred skills like experience with electronic health records and certifications in life support enhance efficiency and preparedness in emergency situations.
