Job Summary:

Premier Valley Medical Group is seeking a compassionate and detail-oriented Licensed Vocational Nurse (LVN) to join our team. In this role, you will support our special programs-Transition of Care, Enhanced Care Management, and COPD Program-while providing oversight to Medical Assistants (MAs) and ensuring high-quality patient care. You will also play a key role in participating in Interdisciplinary Care Team (ICT) meetings, conducting staff training, and helping to develop and improve clinical protocols and workflows to optimize care delivery.


Key Responsibilities:


  • Provide clinical support to the Transition of Care, Enhanced Care Management, and COPD Program, ensuring continuity of care and addressing patient needs.
  • Participate in ICT meetings, providing input and assisting with patient care coordination.
  • Collaborate with providers, case managers, and other care team members to help develop, implement, and update patient care plans as needed.
  • Offer supervision and workflow oversight for Medical Assistants, ensuring efficient operations and adherence to clinical protocols.
  • Monitor and document patient progress, update records, and ensure accurate and timely communication with the healthcare team.
  • Assist in the development and improvement of clinical protocols, workflows, and procedures to ensure best practices and enhance patient outcomes.
  • Conduct patient education on disease management, medication adherence, and self-care strategies.
  • Assist in identifying and addressing barriers to care, including social determinants of health, to improve patient outcomes.
  • Conduct training for Medical Assistants and other staff on workflows, protocols, and clinical best practices.
  • Review charts for accuracy and completeness, addressing any discrepancies and ensuring documentation aligns with clinical standards.
  • Ensure compliance with all regulatory standards and organizational policies.

Requirements:
  • Current and valid LVN license in the state of California.
  • At least 2 year of LVN experience in a clinical setting
  • Strong organizational and communication skills, with the ability to multitask effectively.
  • Knowledge of electronic medical records (EMR) and basic computer skills.
  • Ability to work independently and as part of a team in a dynamic environment.
