Pay Rate: $19.95
The Homeless Outreach Partnership Effort is an outreach team to serve the homeless living on the streets, in abandoned buildings and in camps.
The team will come alongside homeless individuals to understand their needs and link them to ongoing services that will empower them to obtain and maintain stability in the community.
The program is funded through a partnership between the Homeless Services Network of Central Florida. Our PATH program serves the homeless who have mental illness or co-occurring disorders.

What will you do? As a PATH Case Manager at Aspire Health Partners,your job purpose will be to provide a variety of essential functions to coordinatecare for program participants.
Thesefunctions include:
- Providingresource linkage, connection and advocacy to program participants.
- Maintainingup-to-date knowledge of area resources and how to access them.
- Working withmulti-disciplinary teams to gather information and provide the best wraparoundcare possible for clients.
- Documentingclearly and effectively in the Electronic Health Record.
- Following allprogram specific protocols in relation to specified role, maintaining fidelityto program model.
- Maintaining allrequired credentials necessary to perform job functions.
Qualifications
- Bachelor's Degreein Social Services
- 1 year workingwith target population required for some positions
- Pass a Level IIBackground clearance
- Pass a MotorVehicle Records clearance
