We are currently seeking an experienced:

ADMISSIONS AND SOCIAL SERVICES ASSISTANT

Full Time, Days

JOB SUMMARY:

  • Serves as liaison between Admissions Director, families and hospital discharge planners, social service agencies, and physicians;
  • Obtains necessary insurance verifications and prior approvals to guarantee appropriate payment source;
  • Explain costs of care involved in appropriate levels-of-care as determined or required by the admitting physician;
  • When appropriate bed is available and all records are in order, communicates patient admitting schedule according to Facility admissions policy;
  • Notifies the family of admission approval, the room number and patient expected time of arrival;
  • Notifies all appropriate Facility departments of pending admission;
  • Works closely with the Admissions Director and Business Office Manager to ensure continued authorizations are current;
  • Receives and processes prospective resident inquiries and maintains inquiry records;
  • Greets patient and family at door upon admission and assists family and patient and settle into room as needed;
  • Holds meetings with families and/or residents with discharge placement and discharge information as needed;
  • Maintains a list of community resources;
  • Receives and reviews change-of-room requests and notifies appropriate departments of room changes;
  • Works closely with the Admissions Director, Marketing Director, Unit Directors and Social Worker(s) for input regarding admissions and room changes;
  • Other duties as assigned.

 

JOB REQUIREMENTS:

  • Must be a high school graduate. Preferred graduate of a four-year college with one year’s experience in the health care or long-term care field or have an equivalent combination of education and experience.
  • Demonstrated success in working in a healthcare setting. 
  • Must be willing to work and cooperate with other employees.
  • Must be able to problem-solve.
  • Must be able to work under pressure.
  • Demonstrates neat appearance and good personal hygiene.
  • Must read, know and follow personnel, department and facility policies.
  • Must be knowledgeable of local, state and federal regulations as relates to admissions, prospective payment, patient care and social needs of residents.
  • Must have a valid driver’s license in the state of employment.
