About the Role:

As part of Minnie Hamilton Health Systems’ Primary Care Chronic Disease Care Management Program, the Community Health Worker (CHW) is a vital link between healthcare providers and the community, delivering culturally responsive education, advocacy, and hands-on support. This role helps individuals to access care, navigate resources, and build confidence in managing their health to strengthen outcomes across the community.

The Community Health Worker partners with the care team to support high-risk patients through personalized coordination and self-management coaching. Through weekly home or phone visits, the CHW empowers patients to take charge of their health and improve long-term outcomes by building trusting relationships and supporting preventive health initiatives. 

The CHW role collaborates closely with healthcare teams to identify health concerns and connect individuals with appropriate resources such as social services and local organizations to coordinate care. Ultimately, this position aims to empower individuals and communities to achieve better health and well-being through education, outreach, and advocacy.

Responsibilities:

  • Conduct outreach to community members to provide health education and promote wellness programs.
  • Assist individuals in navigating healthcare systems, including scheduling appointments and understanding treatment plans.
  • Identify social determinants of health affecting community members and connect them with relevant social and medical services.
  • Collaborate with healthcare providers and community organizations to develop and implement health promotion strategies.
  • Maintain accurate records of client interactions and report on community health trends to inform program development.

Minimum Qualifications:

  • High school diploma or equivalent.
  • Experience working in community outreach, public health, or a related field.
  • Strong communication and interpersonal skills to effectively engage with diverse populations and build trust within the community.
  • Ability to work independently and as part of a multidisciplinary team.
  • Basic knowledge of healthcare systems and community resources to guide individuals through complex service networks and ensuring they receive appropriate care.
  • CPR certification, proper vital sign measurement
  • Valid driver's license, insurance card and willingness to transport patients.

Preferred Qualifications:

  • Certification as a Community Health Worker or related credential.
  • Interpersonal communication skills including ability to express empathy/compassion
  • Counseling/health promotion skills
  • Experience with electronic health records and data management to support accurate documentation and analysis of community health trends.
  • Training in cultural competency and health education techniques to deliver sensitive and relevant health education tailored to the unique needs of the community.
  • Background in social work, public health, or nursing.
