Case Manager Assistant

Job Description:
- The Case Manager Assistant provides support to Case Managers in coordinating and delivering services to clients.
- The assistant is responsible for helping with administrative tasks, client interactions, and follow-ups to ensure that clients' needs are met effectively and efficiently.
Essential Job Functions:
Administrative Support:
- Assist Case Managers with paperwork, data entry, and maintaining client files.
- Schedule and confirm appointments for clients with service providers.
- Prepare and organize client information for case reviews and meetings.
Client Interaction:
- Communicate with clients to gather necessary information and provide updates on their cases.
- Assist clients in understanding and accessing services available to them.
Coordination of Services:
- Track clients' progress and report any issues or concerns to the Case Manager.
Team Collaboration:
- Work closely with Case Managers and help distribute appointment cards every morning.
- Participate in team meetings and contribute to case discussions and planning.
Minimum Qualifications: Bachelor's degree in a human services field, an equivalent combination of education and experience, or a minimum of two years of experience serving individuals with behavioral health issues. Evidence of experience must be documented in the employee's personnel record. High school or equivalent.
