Position Title: Community Health Worker, Part-time


Position Summary:

Community Health Workers at SC Thrive serve as an essential bridge between communities and the Organization’s partner sites in South Carolina. In this role you will help clients navigate complex social services systems, conduct community outreach, and become a subject matter expert on community resources and federal and state funded programs. You will mix strong technical skills and previous experience in care coordination with a keen understanding of healthcare systems and social drivers of health to effectively support partners and clients. 


This is a part-time, grant funded opportunity with the potential to turn full-time. Must be able to work in Spartanburg County.


Location: Spartanburg County

Full-time/Part-time: Part-time, 25 hours per week

Employment Classification: Exempt, hourly

Compensation: $24-$28 per hour

Travel Requirement: 70-85%

Reporting Supervisor: Director of Strategic Partnerships


Job Duties: 

· Become a trained SC Thrive Counselor with the ability to perform client intake interviews and complete applications as well as determine how to expand and best implement usage of Thrive Hub within partner agencies. 

· Become fully knowledgeable and able to present at numerous speaking engagements on the breadth of SC Thrive work.

· Participate in high leverage state and national coalitions, events, conferences, etc. to promote the work of SC Thrive.

· Become knowledgeable in all aspects of SC Thrive’s client management system Thrive Hub and all other SC Thrive trainings.

· Stay current on industry and organization trends, products, competitive activity, and customer needs.

· Demonstrate excellent proficiency in using integrated technology systems.

· Serve as information source for partners and clients, answering their questions whenever possible.

· Assist clients in evaluating eligibility requirements, education about programs, and enrollment in various benefit programs.

· Build and maintain strong partnerships with healthcare providers and Community Based Organizations (CBOs); conduct community outreach and lead efforts to enhance health literacy and equitable access to care.

· Provide training and technical assistance to partners on CHW workflows, documentation, motivational interviewing, and resource navigation.

· Complete regularly reoccurring reports and documentation related to outreach efforts and actions.

· Establish and maintain effective working relationships with internal teams, healthcare providers, and community partners.

· Actively participate in community outreach events to represent SC Thrive, build relationships with stakeholders, and share SC Thrive approved resources with the public.

· Attend SC Thrive meetings and events as required.

· Other duties as assigned.

Requirements:

Minimum Requirements:

· Bachelor’s degree in health education, health promotion, public health, or a closely related field such as sociology, psychology, social work, or education; or equivalent years of experience.

· Demonstrated experience in care coordination, CHW program implementation, and cross-sector collaboration; knowledge of Medicare/Medicaid billing processes (e.g., CHI, PIN, or SDOH billing) is a plus.

· Strong understanding of healthcare systems, social drivers of health, and integrated care models is essential. Experience working within value-based care or managed care environments is preferred.

· Possesses strong technical skills. Proficient in Word, Excel, PowerPoint, and other technological tools.

· Possesses strong research skills and the ability to conduct internet navigation and research to assist clients.

· Demonstrated desire and ability to work with people from various socioeconomic backgrounds.


Preferred Qualifications:

· Non-profit experience

· Current South Carolina Certified Community Health Worker Certification or actively pursuing certification.

· Strong knowledge of community resources and familiarity with local and statewide health and social services programs, including public benefits and community-based organizations.

· Experience in client advocacy, public health education, or resource navigation.

· Ability to work respectfully and effectively with people from diverse backgrounds and communities.


Additional Notes:

· Pre-employment screens are required per standard policy to include background and drug screening.

· Ability and willingness to work flexible hours, including some nights and weekends, as needed.

· Must have reliable transportation or be able to secure transportation for travel as needed.

· Must have a valid SC driver’s license, minimum state auto insurance, and the ability to travel to multiple worksites in SC.

· Must maintain an acceptable driving record in accordance with SC Thrive policy.

· Must have consistent, secure, high-speed internet access that is at least 25 Mbps download and 4 Mbps upload and meets SC Thrive’s standards.


Working Conditions and Physical Demand:

Most of the time, the job will be performed in the community at partner and potential partner sites, events, rural locations, standard home office, and SC Thrive main office. Requires frequent travel, walking, setting up event space, and often works in outdoor weather conditions. 


Must be able to work with individuals of various diverse backgrounds and socio-economic statuses. Consistently communicates information and ideas to others. Must be able to exchange accurate information in a manner others understand in these situations. Frequently meets with the public and observes details at close range (within a few feet of the observer). Consistently communicates information and ideas to others. Occasionally moves boxes up to 20 pounds for outreach events, materials, and for various purposes.
