The LPN is the heart of our clinic’s operations. In addition to standard clinical support, you will act as a "Patient Navigator" for our most vulnerable community members, ensuring that patients with disabilities receive a seamless, stress-free healthcare experience.

Requirements:

  

• Sensory-Informed Intake: Implement "low-sensory" triage protocols (e.g., reducing noise/lights) for neurodivergent patients to prevent overstimulation.
• Adaptive Clinical Support: Use desensitization techniques and visual aids (Social Stories) to assist patients with autism or cognitive impairments during exams and procedures.
• Accessibility Liaison: Ensure the clinic’s environment and equipment remain ADA-compliant and accessible for patients with limited mobility.
• Caregiver & Community Support: Coordinate with families, group homes, and local agencies to ensure continuity of care and access to respite resources.
• Triage Excellence: Monitor for subtle changes in "baseline" health for patients with communication barriers to identify pain or illness early.

Qualifications:
• Graduate of an accredited LPN program with current State Licensure.
• Current BLS/CPR certification.

• Strong "Disability Etiquette," patience, and the ability to adapt communication to meet the patient’s cognitive and sensory needs.
