About Your Work 

The Quality Improvement (QI) Coordinator provides coordination, reporting, administrative and patient outreach support for various quality improvement initiatives including pay-for-performance (P4P) quality incentives, population health management projects and other FCS QI and Operational programs and projects.  

You will focus on

Payer Incentive Program Coordination:  

  • Oversee various pay-for-performance (P4P) quality incentive programs.  
  • Submit HCC forms and other P4P service documentation to payers for yearly incentive bonuses.
  • Coordinate with clinics (e.g. Providers, Medical Assistants, Front Office) to ensure proper P4P patient documentation for meeting goals, following up with corrections as needed. 
  • Coordinate with clinics to bring patients in for needed P4P services and help increase P4P scores and payments.
  • Support P4P backup documentation in file folders.
  • Support accounts receivable coordination for P4P programs. 
  • Contribute ideas for increasing P4P Care Gap services at FCS

Reporting: 

  • Coordinate, pull and format quality reports from health information technology (HIT) systems as needed for FCS Medical Group and the FCS Family Medicine Residency (e.g. Monthly FCS Quality Stats, Empanelment reports, etc). 
  • Review pulled report statistics, taking note if statistics seem incorrect and bring this up to the proper staff (e.g. Altais/Athena staff, QI Management, Subject Matter Expert). 
  • Work with FCS/Altais/HIT/Subject Matter Experts to troubleshoot or build reports.  

Population Health Management Support:  

  • Call patients or send mass mailers for patient appointments for various P4P incentives or other QI initiatives.  
  • Work on Empanelment projects to repanel Residency panels at the beginning of each academic year as well as FCS Medical Group Repanel projects, when necessary.  
  • Coordination support with other population health management projects as needed.

Administrative Support: 

  • Provider administrative and coordination support for various population health management and quality improvement programs for FCS Medical Group and the FCS Family Medicine Residency.  (e.g. meeting coordination, admin support for meetings, and writing meeting notes)
  • Coordinate and support Patient Advisory Council meetings with Residency leadership
  • Sssist with compliance activities with the ACO and other payers. 

The Skills, Experience & Education You Bring  

  • High School Degree Required
  • Associate of Arts and/or Bachelor degree preferred or equivalent experience.
  • Must be Bilingual in English and Spanish. 
  • Strong written and oral communication skills needed.
  • Detail oriented with data entry, reporting and working with compliance initiatives.
  • Adeptness with Microsoft Office (Excel and Word)
  • Comfort with calling patients for services
  • Comfort and motivation when working with clinic staff and PCPs in order to close the loop on obtaining needed information and corrections on P4P documentation.   
  • Candidates should have an interest in quality improvement and primary healthcare ‘transformation’ subject matter.
  • Desire to do high quality work and support team members as best as possible. 
  • Ability to work independently on projects, showing initiative to maintain momentum and completion of duties on overseen projects.  
  • Flexibility and time management skills to juggle various work duties, prioritizing work based on upcoming or calendared deadlines.

The pay scale listed for this position is generally for candidates that meet the specified qualifications and requirements listed on this job description. Additional pay may be determined for those candidates that exceed these specified qualifications and requirements. Physical Requirements: Office Environment - roles involving part to full time schedule in Office Environment. Based in our physical offices and work from home office/deskwork – Activity level: Sedentary, frequency most of workday.
