About this position
Alliance for Community Transformations is seeking a Case Manager to support individuals participating in the Mariposa Heritage House Adult Drug Court program. This position provides comprehensive case management services to individuals working toward recovery and stability while navigating behavioral health, substance use recovery, and community supervision systems.
The Case Manager works closely with community partners, treatment providers, and program staff to coordinate services, reduce barriers to care, and support participants in achieving long-term recovery and stability. Responsibilities include conducting assessments, developing individualized service plans, coordinating referrals, monitoring participant progress, and maintaining accurate program documentation in compliance with grant and reporting requirements.
This role utilizes trauma-informed, strengths-based, and recovery-oriented practices and plays a key part in supporting participants through the Drug Court process and connecting them to critical community resources.
Requirements:- Bachelor’s degree in social work, Human Services, Psychology, or a related field preferred. Relevant experience may be considered in place of a degree.
- Experience working with individuals impacted by substance use disorders, behavioral health challenges, housing instability, or justice involvement preferred.
- Experience in case management, recovery services, or community-based programs is highly desirable.
- Ability to work collaboratively with community partners, treatment providers, and multidisciplinary teams.
- Strong communication, organizational, and problem-solving skills.
- Ability to maintain professional boundaries and confidentiality when working with sensitive information.
- Must possess reliable transportation, a valid California driver’s license, and DMV clearance.
- Must be able to pass fingerprinting and background clearance.
