JOB SUMMARY:
Responsible for providing psychosocial rehabilitative services to individuals with severe and persistent mental illness in an Adult
Day program.

- Teach and assist members with various activities of daily living skills, (e. g. , shopping, budgeting, food selections, safety skills,
mobility training, etc. ). - Must be able to work in a team environment.
- Must be able to effectively communicate through written and verbal format.
- Performs case management duties which include the procuring of, and linkage to community resources.
- Conducts one-to-one and group sessions related to teaching Activities of Daily Living skill.
- Conducts outreach to clients in the community when needed or indicated on the client’s IRP, in an area conducive to the client’s needs.
- Participates in staff education.
- Participates in policy and procedure improvement initiatives.
- Participates in quality improvement
activities. Assists with care coordination duties for patients (e.g. submitting referrals for support services, scheduling
appointments with PRP, medical follow up, etc.). - Facilitates and assists PHP-A staff with groups and therapeutic activities
- Patient population will include adults and children.
REQUIREMENTS:
- Minimum of 1 year of experience working with behavioral health patients; 3+ years of experience preferred.
- BLS Certification from the American Heart Association required
- Current/Valid Driver's License required
- Successful review by Fleet Safety to operate organizational vehicles
