Job Summary 

The Service Coordinator supports the delivery of high-quality home care services by coordinating client services and supporting daily office operations. This role works closely with clients, caregivers, case managers and internal teams to ensure services are organized, documented and delivered in accordance with care plans and authorizations. The Service Coordinator helps maintain continuity of care while contributing to a positive experience for both clients and caregivers.


Key Responsibilities 

  • Coordinate services for assigned clients to ensure consistent, timely and appropriate care delivery
  • Serve as a primary point of contact for clients, families, caregivers, case managers and referral partners
  • Develop, implement and update individualized service plans based on client needs and authorized services
  • Conduct initial and ongoing client evaluations, both on-site and by phone, to monitor service quality, compliance and satisfaction
  • Manage client intakes, admissions and discharges, including scheduling and preparation of welcome materials
  • Manage client authorizations and support billing accuracy to ensure services are authorized and billable
  • Support staffing and scheduling to ensure shifts are filled, overtime is minimized and service disruptions are addressed
  • Monitor and address missed clock-ins and clock-outs and track EVV data as required
  • Maintain accurate and timely client records in the care management system in accordance with licensing and documentation requirements
  • Provide timely, professional and proactive communication to clients and caregivers, including routine outreach and special occasion communications
  • Support and participate in weekly New Hire Agency Orientation

  • Adhere to all company policies, confidentiality rules, safety requirements and regulatory obligations including HIPAA.
  • Work effectively in fast-paced, dynamic environment, adjusting to shifting priorities and performing the critical day-to-day duties independently with minimal supervision.
Requirements:

 Education and Experience

  • Experience managing and prioritizing administrative responsibilities in an office setting with some time spent in client homes
  • Experience using an electronic CRM or care management system; AxisCare preferred
  • Experience in home care, healthcare or a related service-based industry preferred
  • High School Diploma or equivalent required


Working Conditions

  • Ability to remain in a stationary position for extended periods (office environment)
  • Ability to operate standard office equipment or tools required for the position
  • Ability to move about the work area and between sites as needed


Work Location: Caring for the Coast

Work Hours: Monday – Friday, 8am – 5pm

Supervisory Responsibilities: No

Required Travel: Moderate travel to meet with clients and referral partners
