Young Adult Case Manager

COCOON HOUSE Everett, Washington, United States

About this position

Cocoon House is dedicated to our mission to empower young people, families, and the community to break the cycle of homelessness through outreach, housing and prevention. We believe that every young person deserves a home and the opportunity to achieve their fullest potential. We accomplish this mission through our core values of community, equity, respect, growth, advocacy, leadership, and safety. We do this work because we envision a world where every young person in our community is safe and thriving.

This Case Management position is full time and works in two housing programs: Journeys with Cocoon House and HopeWorks Station II with Housing Hope. Both programs work with 18-24-year-old formerly homeless young adults in a housing first program model. This position is responsible for offering case management to 25-35 formerly homeless young adults, using a strength based, positive youth development approach and helping young adults increase their skills in seven key areas: education, employment, housing, health/wellness, life skills, relationships, and community connectedness. The Case Manager will assist young adults maintaining housing at Cocoon House and potentially making the transition to other permanent housing options when appropriate. Residents accessing housing at Journeys and HopeWorks Station II are not required to access any services, including case management, thus this role requires the staff member to collaborate and build relationships with the young adult in order to establish trust and encourage participation in services when needed. The Case Manager will provide onsite support to help ensure youth are compliant with their lease agreements and are able to find employment.
