We're seeking an experienced and compassionate Licensed Practical Nurse (LPN) to provide Continuous Care for hospice patients. Care is delivered in diverse settings, including private homes, adult family homes, assisted living communities, and skilled nursing facilities. Under the direction and supervision of an RN, the LPN supports the hospice plan of care and collaborates with the interdisciplinary care team to meet the physical, emotional, and spiritual needs of patients and their families.

This role is ideal for nurses who prefer longer shifts, higher earning potential, and the opportunity to provide meaningful, one-on-one patient care.


Schedule and Hours

  • Full-time position with primary responsibility supporting Continuous Care coverage.
  • Continuous Care shifts typically range from 8 to 12 hours
  • Full-time hours may fluctuate based on patient census, eligibility, and care needs
  • Schedule may include days, evenings, nights, weekends, and holidays
  • Flexibility and availability for urgent patient needs are required

Please note: Continuous Care hours are based on patient eligibility and operational needs and are not guaranteed at a fixed schedule.

Additional Compensation

  • Continuous Care Extension Differential: +$5.00 per hour for all unplanned Continuous Care extension hours worked beyond the scheduled shift. This differential is paid in addition to base pay and does not replace applicable overtime pay.


Primary Responsibilities

  • Follow all hospice policies, procedures, and care protocols.
  • Implement care plans under the direction of the RN and IDG team.
  • Provide assigned nursing care, treatments, and preventive procedures.
  • Monitor patient conditions and report changes to the RN and physician.
  • Educate and support patients and families on care needs and symptom management.
  • Assist in evaluating care effectiveness and adjusting plans as needed.
  • Maintain accurate and timely clinical documentation.
  • Attend staff meetings, IDG conferences, and required training.
  • Participate in on-call rotations, including nights, weekends, and holidays.
  • Communicate schedule changes and contribute to interdisciplinary care planning.
  • Assist with procedures, maintain equipment, and observe proper infection control.
  • Support patient and family self-care techniques.
Requirements:


  • Current, active Washington State LPN license in good standing
  • Active BLS certification
  • Valid driver’s license and reliable transportation
  • Minimum 1 year nursing experience in hospice or home health preferred
  • Minimum 2-3 years of healthcare related experience required
  • Proficient with EMR systems and healthcare technologies
  • Ability to exercise sound judgment, initiative, and work independently within scope of practice
  • Adaptable to diverse social, cultural, and religious backgrounds
