About the Role:

The PASRR Case Manager plays a critical role in ensuring that individuals with serious mental illness or intellectual disabilities receive appropriate assessments and services in compliance with the Preadmission Screening and Resident Review (PASRR) federal mandate. This position involves conducting thorough evaluations to determine the level of care needed and coordinating with healthcare providers, facilities, and families to develop and implement individualized care plans. The Case Manager acts as an advocate for the rights and needs of residents, ensuring they are placed in the most suitable care settings and receive necessary supports. This role requires meticulous documentation and reporting to maintain compliance with state and federal regulations. Ultimately, the PASRR Case Manager contributes to improving the quality of life for vulnerable populations by facilitating access to appropriate long-term care services and supports.

Minimum Qualifications:

  • Bachelor’s degree in social work, psychology, nursing, or a related human services field.
  • Experience conducting assessments and case management in a healthcare or long-term care setting.
  • Knowledge of PASRR regulations and federal/state long-term care policies.
  • Strong organizational skills with the ability to manage multiple cases simultaneously.
  • Excellent communication skills, both written and verbal.

Preferred Qualifications:

  • Master’s degree in social work, counseling, or a related field.
  • Certification in case management or related professional credentials.
  • Experience working specifically with individuals with intellectual disabilities or serious mental illness.
  • Familiarity with electronic health records (EHR) and case management software.
  • Training in trauma-informed care or person-centered planning.

Responsibilities:

  • Conduct comprehensive PASRR Level I and Level II screenings and assessments for individuals seeking admission to nursing facilities.
  • Collaborate with healthcare providers, social workers, and family members to develop individualized service plans that address the unique needs of each resident.
  • Monitor and review ongoing care to ensure compliance with PASRR requirements and adjust care plans as necessary.
  • Maintain accurate and detailed records of assessments, care plans, and communications in accordance with regulatory standards.
  • Provide education and guidance to facility staff and families regarding PASRR processes, resident rights, and available community resources.
  • Coordinate with state agencies and other stakeholders to facilitate timely and effective service delivery.
  • Advocate for residents to ensure their needs are met and their rights are protected throughout the care continuum.
