About this position
Position Summary:
The Case Manager supports patients by coordinating care and services, assisting in treatment planning, and providing resources to address mental health and substance abuse challenges. This role involves working closely with patients, their families, and the treatment team to develop individualized care plans that promote sustainable recovery and overall well-being.
Key Responsibilities:
- Conduct initial assessments to determine patient needs, strengths, and available resources.
- Develop and implement personalized case management plans, setting measurable goals and coordinating support services.
- Facilitate access to community resources, including housing, employment assistance, and social services.
- Coordinate with clinical staff to monitor patient progress and adjust care plans as needed.
- Provide ongoing support and guidance to patients throughout their PHP and IOP treatment.
- Document case management activities and patient progress in the electronic medical record (EMR) system.
- Engage families in the treatment process, providing education and resources to support patient recovery.
- Participate in multidisciplinary team meetings, sharing insights and updates on patient progress.
- Ensure compliance with facility policies, regulatory requirements, and ethical standards.
Qualifications:
- Bachelor’s degree in social work, psychology, or a related field (Licensed Social Worker or Certified Case Manager preferred).
- Minimum 2 years of experience in a behavioral health or substance abuse setting.
- Knowledge of PHP and IOP care models and community resources.
- Strong interpersonal, organizational, and communication skills.
- Proficiency in EMR systems and case management documentation.
