Manager, Admissions
QUALIFICATIONS & EXPERIENCE
- A Registered Nurse with a baccalaureate degree in a health care area (or equivalent combination of experience and education), BSN preferred.
- Maintains license as a Registered Nurse from the State of Michigan.
- Two (2) to three (3) years nursing experience in hospice, medical, surgical, or critical care along with one (1) to two (2) years’ experience in leadership or supervisory role.
- Knowledge of advanced illness management and quality end-of-life care preferred.
- Experience working with multi-disciplinary teams, building consensus and leading people and projects forward.
- Possesses and maintains current BLS certification.
- Presents a negative TB skin test and/or chest X-ray and other tests as required by Northstar Care Community.
- Maintains a valid Michigan driver’s license and has the availability of a reliable, licensed and insured vehicle for use on the job, vehicle must be insured in accordance with the State of Michigan.
- Remains up to date on all routine immunizations required by LARA and agency policy.
SUMMARY OF JOB RESPONSIBILITIES
Responsible for the management, planning, coordination and day-to-day operations of all agency clinical care activities provided by the Admissions team.
RESPONSIBILITIES AND DUTIES
Primary Duties
- Reviews and evaluates referrals and admission process. Confers with the Medical Director regarding any questions about an individual’s initial eligibility for service. Ensures eligible clients receive care, barriers to admission are addressed successfully, and patient needs are met or exceeded.
- Provides feedback to staff, ensuring that both effective performance and delivery of quality care are reinforced. Routine field visits with the admission nursing staff.
- Responsible for the schedules for supervised staff and ensure coverage to meet needs including availability to complete admissions as needed.
- Maintains a close working relationship with clinical leadership to ensure appropriate and efficient decisions about prospective admissions.
- Coordinates communication with referral sources concerning all client admissions.
- Coordinates strategies to maximize admissions including daily contact (via phone or outside call) with potential referral sources including but not limited to hospitals, physicians, skilled nursing facilities, assisted living facilities, discharge planners, etc.
- Participates in marketing and education activities regarding Hospice, Grief Support and markets the availability of Hospice and associated services and programs to diverse community populations.
- Promotes exemplary customer service to physicians, facility personnel, contracted partners, referral sources, clients and families at all times.
- Participates in the quality improvement process by identifying customer service issues and reporting these issues to the AVP, Intake and Access.
- Provides assistance with new employee orientation for the admission process.
- Assists the AVP, Intake and Access with identifying, gathering, and interpreting data and other operational and quality indicators for decision making purposes.
- Responsible for EMR workflow related to Admissions activities.
- Provides strong, visible leadership and direction through compelling communication of the agency vision, values, key strategic initiatives, and job expectations.
- Creates an environment of accountability by constructively and appropriately challenging staff to deliver agreed upon outcomes/results and model desired values and behaviors.
- Shares after hours, weekend, and holiday administrator on-call responsibilities.
- Understands and complies with the Medicare Hospice Conditions of Participation.
- Understands and administers agency policies and procedures.
- Responsible for the quality of services and quality initiatives impacting the clinical area. Responsible for the sustainment of the clinical quality initiatives.
- Coaches, mentors, and develop staff and holds them accountable to meet agency/supervisor expectations.
- Participates in financial decision making with CFO or designee for client financial assistance application.
Global Leadership Duties
- Assist staff to develop skill in planning work assignments.
- Attend meetings of the Board of Directors and Standing Committees as requested.
- Provides information, statistics, reports, and records for purposes of evaluation and reporting on departmental activities.
- Responsible for creating a work environment that assists staff in completing tasks in meeting strategic goals.
- Monitors and ensures compliance with applicable local, federal, and state regulations, OSHA, infection control, and Quality Partners standards, compliance guidelines, privacy protections, and protected health information.
Global Leadership & Supervisory Duties
- Works to understand, interpret, adhere to and apply Agency policies and procedures accurately and constructively.
- Provides oversight of responsible areas for the Agency’s budget.
- Hire new staff in accordance with established procedures. Orients or delegates orientation of new employees and ensures that employees have adequate ongoing training and development.
- Establishes and communicates job responsibilities, standards, priorities and expectations to staff.
- Routinely observes and documents both good and poor job performance and conducts timely performance evaluations according to set standards.
- Inform and work in consultation with Human Resources on employee discipline issues (i.e. policy violations, performance issues) and terminations.
- Ensures that staff is properly licensed and/or credentialed.
- Provides oversight of staff hours, competencies, education, and evaluation of the department/area.
