Are you a compassionate and driven Physical Therapist ready to make a difference? At Fellowship Therapy at Home, we’re redefining outpatient therapy by bridging the gap between home health and clinic-based services. We provide care in clients’ homes—delivering therapy where it matters most.
We are seeking a Full-Time Physical Therapist to join our growing team in Somerset County.
This is a full-time position ranging from 30–40 hours per week based on caseload.

What You’ll Do
- Deliver personalized, high-quality outpatient therapy in home and community settings
- Create positive, engaging experiences that empower clients to achieve their goals
- Ensure compliance with documentation and care standards
- Collaborate with a multidisciplinary team to optimize outcomes
- Educate clients and caregivers on safety, exercise programs, and wellness strategies
Qualifications
- Current New Jersey Physical Therapist License (Required)
- Minimum 1 year of outpatient or home health experience (Preferred)
- Strong communication and time management skills
- Ability to travel between client locations throughout the day
- Comfortable working independently
