Director of Rehabilitation (DOR)
Continuum Therapy Partners is seeking a Director of Rehabilitation to lead and oversee therapy services within one of our partner facilities. This role is ideal for a licensed therapy professional who is passionate about leadership, clinical excellence, and team development.
Position Highlights
- Provide leadership and oversight for all rehabilitation services and staff
- Plan, direct, and monitor daily therapy operations to ensure quality outcomes and compliance
- Collaborate with facility leadership to support resident care, census growth, and program development
- Manage staffing, scheduling, productivity, and performance expectations
- Support hiring, onboarding, training, and annual performance reviews for therapy staff
- Ensure timely documentation, billing support, QA processes, and regulatory compliance
- Serve as a clinical and operational resource for the rehab team and facility partners
Qualifications
- Graduate of an accredited PT, PTA, OT, COTA, or SLP program
- Active state licensure required
- Leadership experience in a skilled nursing or post-acute setting preferred
- Knowledge of PDPM/MDS and Net Health (or similar EMR) strongly preferred
Physical Requirements
- Ability to stand, walk, sit, bend, and move throughout the facility for extended periods
- Frequent use of hands for documentation, treatment activities, and computer work
- Ability to assist with or demonstrate therapy interventions, including floor-based activities
- Occasional lifting or transferring of up to 40 pounds using proper body mechanics
- Ability to travel within the facility and, when needed, between facilities
