MPCP is comprised of approximately 100 board certified providers across 10 locations and servicing 130,000 patients. Position Summary:
The Director of Population Health & Quality Improvement provides strategic leadership and operational oversight for MPCP’s population health, quality performance, and value-based care programs. This role is responsible for driving performance outcomes across Maryland’s MDPCP/AHEAD initiatives and commercial value-based contracts while ensuring organizational alignment with quality standards and patient-centered medical home principles.
The Director leads a multidisciplinary team—including case managers, clinical coordinators, social workers, a population health pharmacist, and data analysts—and ensures the effective execution, monitoring, and continuous improvement of all population health and quality initiatives. The position collaborates closely with clinical, operational, and executive leadership to implement data-driven strategies that enhance care delivery, improve outcomes, and optimize reimbursement.
Key Responsibilities:
Strategic Leadership & Program Oversight
- Provide strategic direction and operational leadership for population health, quality improvement, and value-based care programs.
- Oversee departmental operations, staffing, and workflow design to achieve performance targets across all programs and contracts.
- Ensure alignment with patient-centered medical home standards and evidence-based clinical practices.
- Monitor evolving regulatory requirements and industry trends, integrating best practices into organizational programs and workflows.
- Lead or participate in internal and external committees representing population health initiatives.
Value-Based Performance & Program Execution
- Direct implementation and ongoing performance management of MDPCP/AHEAD and commercial value-based contracts.
- Develop and refine case management and clinical workflows to improve care coordination and capture data necessary for payer incentives and program compliance.
- Oversee program compliance with contractual obligations, quality measures, and reporting requirements.
- Identify opportunities to enhance value-based reimbursement through operational improvements and program expansion.
Data Analytics & Performance Reporting
- Analyze clinical, claims, and population-level data to evaluate program performance, identify trends, and uncover opportunities for improvement.
- Design and develop operational and performance reports for executive leadership, clinical teams, and practice operations.
- Present data in clear, actionable formats tailored to different audiences, including providers and administrative leadership.
- Track and report outcomes, successes, and improvement opportunities on a routine basis.
Collaboration & Organizational Integration
- Build strong interdisciplinary relationships across clinical, operational, and administrative teams.
- Collaborate with practice leadership to integrate population health initiatives into daily clinical workflows.
- Partner with payers, CTOs, community organizations and external stakeholders to enhance care coordination, patient resources and value-based initiatives.
- Identify and pursue grant opportunities or partnerships that expand services for MPCP’s patient population.
Qualifications:
Education and Experience
- Bachelor’s degree in health-related field with 3 to 5 years of mid-to senior-level management experience in healthcare, or an equivalent combination of education and experience
- Master’s degree preferred in public health, Health Services Research, Health Policy or Social work or clinical discipline
- Extensive experience using claims data or electronic health record data to identify gaps in care and opportunities to improve outcomes
Skills:
- Knowledge of value-based programs, contracts and population health required.
- Excellent verbal, written and interpersonal communication skills
- Demonstrates strong leadership skills
- Ability to function effectively within an ever-changing environment to meet deadlines and reprioritize as necessary.
- Proven ability to solve problems and make sound decisions.
- Project management skills preferred
- Proficiency in Microsoft applications (Excel, Word and PowerPoint)
- Proficient in electronic health records, CRISP HIE and population health tools (e.g., Epic, Care Management Platforms).
Work Environment:
- Hybrid role with in-office days
- Will require travel to physician practice locations for meetings
